摘要
目的观察丙泊酚复合氯胺酮静脉泵注应用于静脉全麻的效果。方法20例ASAⅠ~Ⅱ级静脉全麻手术患者随机为Ⅰ、Ⅱ组,每组10例。Ⅰ组为丙泊酚及氯胺酮分次静脉注射组,Ⅱ组为丙泊酚及氯胺酮持续静脉泵泵注组。两组术前3min静脉注射丙泊酚0.6mg/kg,氯胺酮1mg/kg。Ⅰ组术中当麻醉转浅时静脉注射丙泊酚(0.6mg/kg)和(或)氯胺酮(1mg/kg);Ⅱ组术中以微泵泵注丙泊酚眼1~3mg/(kg·h)演,氯胺酮眼2~4mg/(kg·h)演。记录麻醉前至手术进行1h每隔5min测得的平均动脉压(MAP)、心率(HR)、呼吸(R)及脉搏氧饱和度(SpO2);整个手术过程中麻醉药用量,术后苏醒时间。结果麻醉期间Ⅰ组MAP、HR、R波动大,Ⅱ组较平稳;Ⅰ组有4例SpO2下降至88%~94%;Ⅱ组所有患者SpO2为98%~100%。Ⅱ组人均氯胺酮及丙泊酚用量明显多于Ⅰ组,两组患者术后苏醒时间无显著性差异。结论氯胺酮及丙泊酚微泵泵注给药麻醉效果好,术中BP、HR和R平稳,安全性好。
Objective To study the effects of propofol combined with ketamine administered by continuous pump-infusion in general intravenous anesthasia.Methods Twenty ASAⅠ-Ⅱpatients undergoing elective burn surgery under general intravenous anesthasia were randomly divided into two groups(ten patients,each):groupⅠand groupⅡ.In groupⅠpatients received propofol and ketamine by intermittent bolus injection and groupⅡby continuons pump-infusion.Three minutes before operation propofol(0.6mg/kg)and ketamine(1mg/kg)were intravenous infused respectively in the two groups.In groupⅠpropofol(0.6mg/kg)and(or)ketamine(1mg/kg)were used by intermittent bolus injection when anesthasia became shallow during operation and groupⅡby continuons pump-infusion(propofol1-3mg/kg·h,ketamine2-4mg/kg·h).Mean arterial pressure(MAP),heart rate(HR)and respiration rate(R)and saturation of puls oxygen(SpO2)were measured each5min from before anesthasia to the end of the first hour.The amount of the two drugs were used during operation and the duration between the end of operation and the patients regained consciousness were calculated.Results MAP,HR and R changed widely in groupⅠ,but were steady in groupⅡ.In groupⅠthere were SpO2of four patients decreased to88%-94%,but it was stabilized to98%-100%.The amount of the two drugs with everyone was significantly less in groupⅠthan in groupⅡ.Duration between the end of operation and the patients regained consciousness was not significantly different between the two groups.Conclusion The effects of propofol combined with ketamine administered by continuous pump-infusion in general intravenous anesthasia are satisfactory;BP,HR and R are stable during the whole course.This anesthetic method is safe.
