人工关节松动的生物学因素:细胞因子和人工关节骨溶解 被引量:4

摘要 影响人工关节远期寿命的最主要原因是远期松动。而大量的证据表明松动的主要元凶是磨损颗粒引起的假体周围的骨溶解。在骨溶解过程中,细胞因子对成骨细胞、破骨细胞的发生、分化、成熟、死亡起着重要的调节作用。这些细胞因子主要包括:白介素1,肿瘤坏死因子(TN)F,前列腺素等。磨损颗粒释放入关节腔后,首先被巨噬细胞吞噬巨噬细胞不仅本身分泌因子作用于成骨细胞、破骨细胞,其本身可能也参与骨溶解过程。解决骨溶解的基本策略是减少交界组织处磨损颗粒的数目和增加骨-植入物界面的完整性。通过药物可以阻止某些细胞因子如TNF的分泌,进而达到预防无菌性松动的目的。
