摘要 目的:探讨胃肿瘤病人术后早期应用肠内营养(enteralnutrition,EN)的可行性、安全性及临床疗效。方法:在20伴营养不良的胃部肿瘤病人中,采用术后早期EN,分别在营养支持前,后测定体重(BW)、体质指数(BMI)、血浆白蛋白(Alb)、前白蛋白(PreA)、转铁蛋白(TFN)、免疫球蛋白、淋巴细胞计数、C反应蛋白(CRP)、肝肾功能、胃泌素(GAS)、胃动素(MTL)、胆囊收缩素(CCK)水平,同时观察者的生命体征、胃肠道功能等恢复状况,以及有无胃肠道不良反应和术后并发症。结果:所有患者均可安全耐受了术后早期肠内营养,无严重不良反应。病人Alb、PreA、免疫球蛋白、细胞免疫功能、GAS、MTL水平于术后第7天即恢复正常,TFN、CCK水平则较术前明显升高。结论:胃肿瘤术后采用早期EN,而且安全、可靠,能即期改善病人营养不良状态,并缓解肿瘤和手术创伤所导致的免疫功能紊乱,有利于恢复护胃肠道功能。 Objective To explore the safety, feasibility, clinical effects and cost-effectiveness of early post-operative enteral nutrition with dietary fibers in gastric tumor patients. Methods Twenty patients with gastric tumor suffering form malnutrition were given early post-operative enteral nutrition with dietary fibers. Before and after the nutritional support therapy, the body weight, body mass index, albumin, prealbumin, transferrin, immumoglobulins, lymphocyte count, C-reactive protein, the renal and liver functions, as well as the levels of gastrin, motilin and cholecystokinin were tested. The vital sign, recovery of gastrointestinal funtions, gastrointestinal side effects and complications were oberserved at the same time. Results No serious complications occurred in all the patients during the study, They all received early post-operative enteral nutrition uneventfully. By the 7th post-operative day, the serum levels of albumin, prealbumin, immumoglobulins, gastrin and motilin as well as the cellular immunity functions became normal. The levels of transferrin and cholecystokinin increased significantly. Conclusions Early post-operative enteral nutrition with dietary fibers is safe and feasible in gastric tumor patients. The therapy is helpful in improving the overall nutritional status, in early resumption of post-operative gastrointestinal function, and in evasion of immune dysfunction caused by tumor and surgery.
