摘要
[目的 ]探讨长期机械通气老年病人呼吸机管道细菌污染状况。[方法 ]对 60例次长期机械通气老年病人的呼吸机管道及湿化器在使用前及连续使用 7d后进行细菌培养 ,同时采集下呼吸道分泌物做痰培养。 [结果 ]呼吸机管道和湿化器在使用前无细菌生长 ,使用 7d后细菌阳性率达 10 0 % ,管道中细菌均数为 4.2× 10 4cfu/cm2 ;湿化水中细菌均数为 63× 10 4cfu/mL。从管道与痰标本中共分离出细菌 14 1株 ,革兰氏阴性杆菌 12 3株 ,占87.2 3 % ,其中铜绿假单胞菌居首位 ,占 45 .3 9% ,其次为大肠埃希菌18.44 %。同一病人呼吸机管道内细菌与痰培养结果存在一致性。[结论 ]机械通气老年病人呼吸机管道及湿化器污染严重 ,其病原菌种类与痰培养结果一致 ,铜绿假单胞菌最常见。
Objective: to probe into the status quo of bacterial contamination of tubes of ventilator for senile patients with long-term mechanical ventilation. Method: a total of 60 cases/times of bacteria culture of tubes of ventilator and humidification device was performed before used and 7 days after use continuously for senile patients with long-term mechanical ventilation. And lower respiratory tract secretions of all patients were cultured as well. Result: no bacteria were found from tubes of ventilation and humidification device before used. However it was found bacteria from all these devices seven days after using. The average bacterium colony forming units (CFU) were 4.2×10~4 cfu/cm^2 in tubes of ventilator, and 63×10~4 cfu/mL in the water of humidification devices. Totally 141 strains of bacteria were detached from either tubes of ventilator or sputum of patients.Among which 123 strains were Gram's-negative accounted for 87.23%. Pseudomonas aerugo was in the first place of these Gram-negative bacteria, which accounted for 45.39%. And Escherichia coli was in the second place accounted for 18.44%. There was a consistency between bacteria culture of ventilator tubes and sputum culture of patients. Conclusion: both ventilators and humidification devices were badly contaminated in senile patients accepted long term mechanical ventilation. The kinds of bacteria cultured from these devices were similar to those from sputum culture of the same patients.Pseudomonas aerugo was the most common bacterium.
