摘要
目的探讨吸烟对 2型糖尿病患者糖代谢的影响。方法用临床化学分析仪测定 2型糖尿病患者吸烟组 (n =1 2 8)与非吸烟组 (n =1 2 0 ,对照组 )的血糖和氨基果糖 (FA) ,放射免疫法测血清胰岛素 ,用稳态模型分析法 (HOMA)评价胰岛素抵抗 (IR)。结果吸烟组与对照组患者空腹血糖 (FPG)水平分别为 (1 0 .0 6± 3.2 6 )mmol/L和 (9.5 0± 3.34)mmol/L(P >0 .0 5 ) ;餐后 2h血糖 (2hPG)分别为 (1 5 .97± 5 .0 0 )mmol/L和 (1 4 .6 4± 4 .71 )mmol/L(P <0 .0 5 ) ;葡萄糖曲线下面积 (AUCglu)分别为 (2 9.2 5± 1 0 .2 2 )mmol/ (L·h)和 (2 5 .1 0± 8.82 )mmol/ (L·h) (P <0 .0 1 ) ;FA分别为 (2 .82± 0 .90 )mmol/L和 (2 .5 0± 0 .75 )mmol/L(P<0 .0 5 ) ;HOMA IR分别为 4 .88± 2 .2 0和 4 .2 1± 1 .83(P <0 .0 5 )。结论吸烟对 2型糖尿病患者的 2hPG、AUCglu和FA均有显著影响 。
ObjectivesTo explore the effects of smoking on glucose metabolism in patients with non insulin dependent diabetes mellitus(NIDDM).MethodsTwo hundred and forty eight NIDDM patients were divided into smoking group (n=128) and non smoking group(n=120,control group). Their plasma glucose and fructosamine (FA) levels were estimated by clinical biochemical analyzer; serum insulin levels were detected by radioimmunoassay(RIA) and insulin resistance(IR) was evaluated by homeostasis model analysis (HOMA).ResultsFasting plasma glucose(FPG)levels in smoking and control groups were 10.06±3.26 mmol/L and 9.50±3.34mmol/L, respectively (P>0.05); 2nd h postprandial plasma glucose (2 hPG) levels in both groups were 15.97±5.00mmol/L and 14.64±4.71 mmol/L separately (P<0.05); their area under glucose curve (AUCglu) were 29.25±10.22mmol/(L·h) and 25.10±8.82mmol/(L·h)(P<0.01) as well as their plasma FA levels were 2.82±0.90 mmol/L and 2.50±0.75 mmol/L, respectively (P<0.05). HOMA IR in smoking and control groups were 4.88±2.20 and 4.21±1.83, respectively (P<0.05).ConclusionSmoking affects significantly the 2hPG, AUCglu and FA in NIDDM patients, and these effects seem to be related to the increase of insulin resistance .
