摘要
目的 探讨海德堡视网膜断层扫描仪 (HRT)检测开角型青光眼视盘参数与视野损害的关系。评价HRT在早期诊断青光眼中的意义。方法 正常人 2 6例 (4 1只眼 ) ,高眼压 1 1例 (1 6只眼 )、原发性开角型青光眼 2 8例 (38只眼 )。采用 Humphrey全自动视野计、HRT分别进行视野、视盘形态检测。比较正常组、高眼压组、青光眼组HRT视盘检测参数 ,分析青光眼组视野检测的平均缺损 (MD)与 HRT视盘检测参数的关系。结果 正常组、高眼压组、青光眼组视杯面积、杯 /盘面积比、盘沿面积、视杯形态测量指数、视网膜神经纤维层厚度差异有显著性。青光眼组的盘沿面积、杯 /盘面积比、视杯形成测量指数、视网膜神经纤维层厚度与视野检测的平均缺损有显著相关。结论 HRT能够反映青光眼视盘改变 。
Objective To evaluate the relationship between visual field and the topographic parameters of optic discs measured by Heidelberg Retinal Tomography and to access the significance of HRT in the early diagnosis of primary open angle glaucoma.Methods The topography of optic discs was quantified by HRT in 26 normal controls (41 eyes),11 patients(16 eyes) with ocular hypertension and 28 patients with POAG(38 eyes).Topographic parameters were compared among all groups.The correlation between mean devation and topographic parameters in glaucomatous eyes was also analyzed.Results Among the HRT parameters,the mean values for cup areas,cup disk area ratio rim area,cup shape measure indics and retinal nerve fiber layer thickness showed significant differences among all groups.Meanwhile,a significant relationship existed between the mean deviation and cup disk area ratio,rim area,cup shape measure indices,retinal nerve fiber layer thickness in glaucomatous eyes.Conclusions Measurement of topographic parameters by HRT is useful in detecting early glaucomatous damage.Furthermore,HRT measurement of topographic parameters may provide clinically relevant information in monitoring glaucomatous changes.
