摘要 目的 探讨多层螺旋CT增强扫描对肾脏肿物检出率及其特征 ,并提出适于肾脏肿物特征研究的肾脏螺旋CT扫描模式。资料与方法  115例患者行肾脏 4期螺旋CT增强扫描 ,统计不同期的检出率及其与病灶大小和部位的关系 ,并分析肾脏实性肿物的强化程度和最大强化值出现时相。结果 共有 173个病灶 ,其中囊性 14 6个 ,实性 2 7个。实质期和排泄期的病灶的检出率均为 10 0 % ,直径 >2cm的病灶的检出率为 10 0 %。直径 <2cm的病灶的检出率的高低与扫描时相和病灶部位有关。不同肾脏实性肿物的强化方式不同 ,大多数肾细胞癌表现为重度强化 ,其最大强化值出现在皮髓质期。结论 实质期是检出小的肾脏肿物的最可靠的时相 ,皮髓质期是肾脏肿物特征分析的重要时相 ,肾脏肿物的检出和特征研究宜采用肾脏 Objective To investigate the detecting rate of renal mass on enhanced multi-slice helical CT scans, and to discuss the CT features of the lesions.Materials and Methods Contrast-enhanced tetra-phasic helical CT scanning of the kidney was performed in 115 patients. The detecting rate of renal lesions in each phase was calculated, and its relation with lesion's size and location was analyzed. The enhancement degree of the solid lesion and the time of maximum enhancement value were observed.Results A total of 173 lesions was found, including 146 cystic and 27 solid lesions. All the lesions could be detected during both nephrographic and excretory phase. The detecting rate for the lesions with the diameter larger than 2cm was 100%, while the detecting rate for the lesions less than 2cm was related to the scanning phase and lesion's location. Different solid renal lesions showed different enhancement patterns. Most renal cell carcinomas showed marked enhancement with the peak value appearing in cortico-medullary phase.Conclusion Nephrographic phase is the most reliable phase for the detection of small renal mass. Cortico-medullary phase is the best phase for analyzing lesion's characteristics. For the detection of renal mass and for the observation of the lesion's feature, contrast-enhanced tetra-phasic helical CT scanning of the kidney should be adopted.
