右心构型无改变SLE患者肺动脉血流的超声检测价值 被引量:5

Ultrasonographic Value for Evaluafion of Pulmonary Arterial Flow in SLE Patients Without Right Heart Geometric Model Changes

摘要 目的 探讨右心构型无改变的系统性红斑狼疮 (SL E)患者肺动脉血流的变化 ,为 SL E并发肺动脉高压的早期临床诊治提供有价值的信息。方法 应用多普勒超声心动图检测 18例右心构型无改变的 SL E患者和 2 0例健康对照组的肺动脉血流参数 ,包括肺动脉血流加速时间 (AT)、右室射血前时间 /加速时间 (PEP/ AT)、右室射血前时间 /射血时间 (PEP/ RVET)。结果  SL E患者的 AT、 RVET缩短 ;PEP延长 ;PEP/ AT与 PEP/RVET比值增大 ,其与对照组的比较结果均有显著性差异 (p<0 .0 5 ) ,其中以 AT与 PEP/ AT两组间的差异性尤为显著 (p<0 .0 0 1)。结论  SL E患者在右心构型无改变时 ,肺动脉血流已发生变化 ,提示其肺动脉压趋于增高。多普勒超声心动图作为一种无创的检查手段可对其进行准确检测 ,从而为病情判断及诊治提供重要信息。 Objective To evaluate the pulmonary arterial flow in SLE patients without right heart geometric model changes and in order to supply valuable informations for earlier clinical diagnosis and treatment of pulmonary arterial hypertension.Methods Pulmonary arterial flow parameters in 18 SLE patients and 20 healthy controls were measured using Doppler echocardiography,including AT、PEP/AT、PEP/RVET.Results AT and RVET were decreased,PEP prolonged,PEP/AT and PEP/RVET increased in SLE patients which had significant differences compared with those of healthy controls(P<0.05),especially AT and PEP/AT were significantly different between patients and the control group (P<0.001).Conclusions Pulmonary arterial flow was obviously changed in SLE patients without right heart geometric changes,which meant that the pumonary arterial pressure had alrady an increasing trend,earlier than the geometric changing.Doppler echocardiography as a noninvasive and accurate examination method can be used to supply important informations for judging,estimating,diagnosing and evaluating the treatment effect of pulmonary hypertension.
