摘要
目的 对人血清N端脑钠肽 (NT proBNP)水平参考值范围及其测定稳定性进行初步调查。方法 采用电化学发光法 ,测定 10 2名健康成年人血清NT proBNP水平 ,分析各项生理指标对NT proBNP的影响 ,建立NT proBNP的参考值范围 ,并对抗凝剂和蛋白酶抑制剂对NT proBNP测定的影响以及NT proBNP在不同温度下的稳定性进行初步探讨。结果 NT proBNP随着年龄增大而升高 ,二者相关 (r=0 331,P =0 0 0 1)。女性NT proBNP水平 (6 1 5± 2 7 2 )明显高于男性 (32± 2 2 3,P <0 0 1)。以第 97 5 %位数为参考值上限 ,5 0岁以下 ,男性为 5 6pg/ml,女性为 95pg/ml;5 0岁以上 ,男性为 98pg/ml,女性为 116pg/ml。不同采血管测定 ,与血清组比 ,Aprotinin组略高 ,是血清组的 1 0 16倍 (r =1 0 0 0 ) ,EDTA组则比血清组低 7 0 6 % (r=1 0 0 0 )。 2 5℃NT proBNP活性可稳定 3天 ,4℃可稳定 5天 ,- 2 0℃和 - 70℃NT proBNP至少可以稳定 1个月。结论 年龄、性别是影响人血清NT proBNP水平的重要生理因素 ,应根据不同年龄和性别设立NT proBNP的参考值范围。NT proBNP测定可用EDTA抗凝管或无抗凝剂的采血管 。
Objective To investigate reference range and measurement stability of human serum NT-proBNP. Methods An automated electrochemiluminescence immunoassay was used to measure concentration of serum NT-proBNP on Roche Elecsys 2010. Totally 102 cases of healthy adults were tested. The impact of age, sex and other physiologic characteristics on NT-proBNP was analyzed. Then reference range was established. The influence of anticoagulants or protease inhibitor on NT-proBNP measurement was evaluated. The stability of NT-proBNP at different temperatures was appraised too. Results NT-proBNP had a positive correlation with age( r =0 331, P =0 001). Serum level of NT-proBNP was higher in women(61 5±27 2) compared to men(32±22 3, P <0 01). The 97 5% upper limit of reference is <50 years 56 pg/ml for men, 95 pg/ml for women and > years 98 pg/ml for men, 116 pg/ml for women. Compared with serum, NT-proBNP in serum with aprotinin is 1 016 times higher( r =1 000), and in EDTA plasma is 7 06% lower( r =1 000). NT-proBNP was stable at 25℃ for at least 3 days, 4℃ for 5 days and could be extended to at least 1 month when stored at -20℃ or -70℃. Conclusion Age and sex are two important physiologic characteristics which have an impact on human serum NT-proBNP level. Establishment of NT-proBNP reference range should take gender and age into consideration. NT-proBNP assay could use EDTA plasma or serum, and its immunoreactivity was stable at different temperatures.
