摘要
目的:探讨多层螺旋CT(MSCT)后处理重建技术在肺动静脉畸形(PAVM)中的临床应用价值。材料与方法:回顾分析15例PAVM的MSCT三维肺血管成像表现,其中9例经手术病理证实,6例经DSA及临床随访证实。结果:全部病例三维肺血管图像均能充分显示引流肺动、静脉的空间关系与解剖细节,可以对肺动静脉畸形病人做出明确诊断;能准确、直观地观察引流肺动脉的走行、数目、直径,以指导介入治疗。结论:瘤样扩张的血管囊腔与其相连的供血动脉和引流静脉血管为PAVM的基本构架,MSCT后处理重建技术在肺动静脉畸形的诊断和评价中是理想和无创伤的影像学方法。
Objective: To evaluate the value of three-dimensional reconstruction of multi-slice CT for pulmonary vessels and its clinical application in pulmonary arteriovenous malformation. Methods: Multi-slice spiral CT angiography was performed in 15 pulmonary arteriovenous malformation. The imaging manifestations of 9 cases were comfirmed by operative pathology, the other 6 cases were diagnosed by DSA and typical clinical manifestations. Results: Three-dimensional images of all patients could show space relationship and anatomical characters of pulmonary input arteries and output veins clearly diagnose pulmonary arteriovenous malformation, and can accurately and directly reveal the track, number and diameter of pulmonary input arteries to guide interventional therapy. Conclusion: The tumor-like dilatation of the vascular sac connected with the feeding artery and drainage veins was the basic structure of PAVM. Three-dimensional images play an important role in diagnosis and future treatment in pulmonary arteriovenous malformation.
