摘要
目的 :探讨采用眶周注射曲安奈德联合维生素B12 对Graves眼病的疗效。方法 :对 1999～ 2 0 0 3年间在我院就诊的病史不足 1年的Graves眼病患者共 38例 ,男性 9例 ,女性 2 9例进行局部眶周注射曲安奈德及维生素B12 ,观察Graves眼病阳性体征改变情况 ,以探讨其疗效。甲状腺机能障碍分别加用相应的药物。结果 :治疗 5个疗程以后 ,患者眼球运动及复视不同程度地改善 ,眼突度明显下降 (P <0 0 1) ,睑裂宽度明显恢复 (P <0 0 5 ) ,眼睑迟落得以改善。眼压恢复或控制。影像学检查发现肌腹肥大减轻或消失。 32例患者的角膜荧光染色检查全部呈现阴性 ,均已不着色。该治疗对于视力及视野无明显改善作用。治疗 5个疗程以后 ,没有明显全身毒副作用。结论 :虽然Graves眼病目前尚无特殊治疗的药物 ,但采用眶周注射曲安奈德及维生素B12 对改善眼部症状、减少眼球突出及眼睑后退等方面有一定的疗效 ,并且克服了长期全身应用激素所造成的毒副作用。
Objective:To explore the effectiveness of Triamcinolone Acetonide(TA) and Vitamine B 12 for Graves ophthalmopathy by peri-orbit injection.Methods:In year 1999～2003,38 cases(male 9 and female 29)of Graves ophthalmopathy patients with history no lnger than one year in our hospital were treated with TA and Vitamine B 12 by peri-orbit in jection and evaluate its effects.If necessary the relevant thyro-malfunction drugs were accompanied.Results:The ocular movement and diplopia improved greatly.The exophthalmos degree decreased P<0.01.The palpebral fissure width turned to normal,P<0.05.The lid lag improved also.And the intraocular pressure turned to normal or was under controlled; The image analysis found that the hypertrophy of muscle belly decreased or even disappeared.The cornea fluorescence staining took negative reaction in all 32 cases.However the treatment took no effect on visual acuity and visual field.After 5 courses of treatment there was no obvious poisonous and side effects in the whole body.Conclusions:Triamcinolone Acetonide(TA) and Vitamine B 12 by peri-orbit injection is effective for Graves ophthalmopathy.Other than that there is no side effect on the whole body.
