摘要
目的:为了探讨甲基强的松龙(MP)和神经节苷脂(GM-1)在急性脊髓损伤中继发性损伤的疗效观察。方法:本组患者共31例。随机分为2组,A组15例静脉给予大剂量MP治疗,B组16例静脉给予GM-1治疗,用药时间A组在8h之内,B组在72h之内。结果:本组患者全部随访1~3年,平均2.5年,按照总恢复率和有用恢复率进行比较,应用MP组总恢复率1周为66.7%,半年为73.3%,1年为80%,GM-1总恢复率1周为25%,半年为43.7%,1年为56.3%,有用恢复率MP为46.7%,GM-1组为31%。依据统计学分析,两者1周总恢复率有显著性差异(P<0.05),两者1年总恢复率与有用恢复率无显著性差异(P>0.05)。结论:正确及时的使用MP对早期脊髓继发性损伤有良好的预防作用。明显优于GM-1且持久,GM-1可以促进残余神经的恢复。
Objective:To study the medicinal effects of MP and GM-1 gangliosioside for the patients with secondary injury caused by acute spinal cord damage. Methods:31 cases of the patients were divided into two groups randomly. A group was used high does of MP during 8 hours and B group used GM-1 during 72 hours after injury. The total recovery rates were compared between two groups in one week, a half year and one year respectively. Results:All the patients were obtained 1-3 years of follow-up examination. The total recovery rates of A group and B group were 66.7%, 25% ( one week ), 73.3%, 43.7% (a half year) and 80 %, 56.3 %( one year) respectively. There was a significant difference between the A and B group in ASIA score one week after therapy (P<0.05), but there was not significant difference between two group 1 year after treatment(P>0.05). Conclusion:This study provide evidence that high does of MP used timely can enhance the recovery of neurologic function of early secondary spinal cord injury and its effect is better than GM-1. GM-1 can improve the recovery of residual neurologic function.
