摘要
目的 评价FDGPET鉴别脑肿瘤治疗后复发和坏死的价值。方法 3 2例临床怀疑复发的恶性脑肿瘤病人 ,18例脑胶质瘤 ,14例脑转移瘤 ,分别于治疗后 6~ 2 3个月行FDGPET显像 ,图像分析采用目测法 ,将FDG摄取分为 3级 ,≥ 2级视为复发。最后诊断以病理活检 (18例病人经过病理活检 )或CT、MRI检查以及长期临床随访为准。结果 PET检出的 2 1例复发病灶中 ,2 0例为真阳性 ;11例坏死病灶中 ,6例为真阴性。灵敏度、特异度分别为 80 %、86%。
Objective To evaluate the value of FDG PET in differentiation of brain malignant tumour recurrence from radiation necrosis. Methods PET scan was performed on 32 patients (18 gliomas and 14 brain metastatic tumors) with clinically suspected of brain tumor recurrence after radiotherapy 6 to 23 months. The final diagnosis was confirmed by histopathological finding in 18 cases and CT, MR imaging and clinical follow-up in 14 cases. Visual analysis was used in PET image interpretation and FDG uptakes in tumors were graded into three levels: level Ⅱ and Level Ⅲ were considered as the evidence of tumor recurrence and Level I as radiation necrosis. Results Twenty-one recurrent foci and 11 radiation necroses were shown in PET images. In 21 recurrent cases, 20 were true positive, while in 11 necrotic cases, 6 were true negative. The sensitivity and specificity of PET for detecting tumor recurrence were 80% and 86%, respectively. Conclusion FDG PET is a powerful tool in differentiating brain tumor recurrence from radiation necrosis after radiotherapy.
