摘要
目的 研究新的凋亡抑制因子survivinmRNA在肝细胞癌 (hepatocellularcarcinoma ,HCC)中的表达 ,测定肝癌的细胞凋亡和增殖及多药耐药基因mRNA在HCC中的表达 ,探讨survivin表达对肝癌细胞凋亡和增殖的影响及其与HCC多药耐药之间的相互关系。方法 31例HCC癌组织及癌旁组织标本取自 2 0 0 0年 8月至 2 0 0 1年 4月浙江大学医学院第二附属医院。用逆转录聚合酶链反应(RT PCR)、TUNEL法和免疫组织化学法分别测定HCC癌组织及癌旁组织中survivinmRNA的表达情况、细胞凋亡指数和增殖指数及MDR 1mRNA的表达 ,分析在HCC中survivin表达与细胞凋亡和增殖之间的相互关系 ,探讨survivin表达在HCC发生发展中的作用及其与MDR 1mRNA表达之间的关系。结果 31例肝癌组织中survivinmRNA阳性例表达率为 71% (2 2 / 31) ,阴性表达率为 2 9% (9/ 31) ;而癌旁正常组织中均无表达。肝癌标本中的凋亡指数为 :0 4 %~ 15 % ,平均值为 :(4 2 4± 3 92 ) %。Sur vivinmRNA阳性表达病例平均AI值为 (4 6 4± 4 30 ) % ,阴性表达平均AI值为 (3 36± 2 98) %。两者比较P >0 0 5 ,两组间无显著差异。肝癌标本中增殖指数为 :2 %~ 80 % ,平均值为 (2 6 2 1± 14 9) %。SurvivinmRNA阳性表达病例平均Ki 6 7LI值为 (30 32± 15
Objective To study the mRNA expression of survivin, a novel apoptosis inhibitor, and cell apoptosis and proliferation and MDR 1 in hepatocellular carcinoma (HCC) and investigate the effects of survivin mRNA expression on apoptosis and proliferation and MDR 1 in HCC. Methods RT PCR, TUNEL and SABC IHC were used to detect the expression of survivin mRNA and MDR 1 mRNA, cell apoptosis and the ki 67 expression, respectively, in samples from 31 cases of HCC. Meanwhile, the effects of survivin mRNA expression on cell apoptosis and proliferation and the relationship between survivin mRNA expression and MDR 1 mRNA expression were analyzed. Results The positive survivin mRNA expression was observed in 22 out of the 31 cases (71%). All the neighboring non tumor and cirrhotic tissues were completely negative for survivin mRNA expression. The apoptotic index (AI) was 0 4%~15% and the overall average was 4 24±3 92%. The average of AI was 4 64±4 30% in survivin mRNA positive cases and 3 36±2 98% in negative cases. There was no significant difference between the 2 groups of patients (P>0 05). The ki 67 proliferation index (LI) was 2%~80% and the overall average was 26 21±14 9%. The average LI was 30 32±15 43% in the survivin mRNA positive cases and 17 56±9 44% in negative ones. The LI showed a significant positive correlation with the survivin mRNA expression (P<0 05). The positive MDR 1 mRNA expression was 18/31 (58 05%) in HCC tissue. Amongst the 22 survivin mRNA positive cases, 15 were MDR 1 mRNA positive and 7 negative. As for the 9 survivin mRNA negative cases, 3 were MDR 1 mRNA positive and 6 negative. Conclusions Survivin may play an important role in the progression of HCC. There is no significant difference between survivin mRNA expression and AI. However, LI shows a significant positive correlation with the survivin mRNA expression. Though survivin is an apoptosis inhibiting agent, it does not significantly influence cell apoptosis in HCC, and it plays an important role in the progression and development of HCC by promoting cell proliferation. Survivin will be a potential new target of gene therapy in HCC. There is no significant correlation between survivin and MDR 1 expression. The formation of MDR in HCC is attributed to various kinds of pathway but not survivin.
