摘要
目的 研究上颈椎外伤的多层CT扫描技术及其价值。资料与方法 回顾性分析 2 7例上颈椎外伤 ,其中外伤在 1个月内者共 18例 ,1个月以上者 9例。采用 4层螺旋CT扫描仪 ,1.0mm准直 (1.3mm有效层厚 ) ,5 0 %重叠重建 ,并进行多平面重建 (MPR)、三维表面遮盖显示 (3D SSD)和容积显示 (VR)成像 ,分别评价骨折、关节脱位以及韧带损伤等。结果 CT清楚显示上颈椎外伤的骨折情况 ,以原始横断面图像和MPR图像结合显示最佳 ,并可检出齿状突水平骨折和齿状突先天变异 ,但陈旧性 (1个月以上 )外伤单纯横断面显示常不完善 ,至少需要MPR的补充显示 ;关节脱位的显示以VR或SSD为佳 ,寰椎横韧带损伤表现为横断面上齿状突两侧韧带结构不对称 ,VR横断面薄层块显示更好。结论 1.0mm准直扫描结合MPR和VR图像是目前 4层CT机作上颈椎外伤检查的上佳技术组合 。
Objective To study the technique and the value of mulitslice CT (MSCT) in upper cervical trauma.Materials and Methods 27 patients with upper cervical trauma including 18 cases with trauma history within 1 month, 9 cases more than 1 month, were analysed retrospectively with MSCT scan, 1 mm collimation, 50% overlapping reconstruction, and multiplanar reconstruction (MPR), 3-dimensional shaded surface display (3D SSD) and volume rendering (VR) were applied to analyse fracture, dislocation, and ligament injury. Results The fracture was clearly displayed, especially in the axial and MPR images, and the horizontal fracture and variance of odontoid were also detected on these images. But the display of old trauma(>1 month) is not satisfied on axial image, which need supplementary information with MPR. The dislocation was well shown in VR or SSD, and the atals transverse ligamentous injury was shown as asymmtry of odontoid ligament structure in axial and more clearly depicted with thin slice VR images.Conclusion 1 mm collimation combined with MPR and VR was the optimal technique in upper cervical examination with 4 slice CT scanner, and it could reveal the condition of upper cervical trauma.
