摘要
目的 探讨同步多频听觉稳态诱发反应在小儿听力诊断中的应用。方法 39例受试者,年龄6个月~5岁,平均2.75岁,男21例,女18例。分别进行同步多频听觉稳态诱发反应和小儿行为听力测试。同步多频听觉稳态诱发反应刺激声信号的载波频率为0.5 kHz 、1 kHz 、2 kHz及4kHz。双耳上述各个频率分别以70~100 Hz不同的调制频率进行调幅调制。测试时,双耳八个(每耳四个)声信号经ER-3A插入式耳机同步给出。测试状态为水合氯醛镇静睡眠。小儿行为听力测试中,6~24月龄小儿行视觉强化定向条件反应测听、25月龄~5岁行游戏测听。所有数据以SPSS统计软件分析。结果 ①小儿行为听阈通常比同步多频听觉稳态诱发反应阈低,在0.5kHz、1kHz 、2kHz及4kHz二者差异的平均值及标准差分别为:15.2±9.1dB、8.8 ±7.5dB、8.2±7.7dB、10.7±8.8dB。各个频率之间这一差异存在统计学意义(F =10.395;μ 3 ;P=0.000),进一步的两两比较显示0.5 kHz行为听阈与反应阈的差异高于其它频率;②小儿行为听阈与同步多频听觉稳态诱发反应阈在各个频率均呈显著相关(P =0.000)。Pearson相关系数分别为0.799,0.859,0.894,0.850。结论 同步多频听觉稳态诱发反应测试具有频率特异性、客观及与行为听阈相关性好的特点。
OBJECTIVE To explore the use ofauditory steady-state response (ASSR) to mul-tiple simultaneous stimuli for threshold estima-tion in children.METHODS The subjects were 39children with variant degree of hearing loss. Theirage ranged from 6 months to 5 years with the av-erage of 2.75. The equipment was SMART ASSRfrom the INTELLIGENT HEARING system. Simul-taneous carrier tones (0.5 kHz, 1 kHz, 2 kHz and4kHz), modulated in amplitude at different rate of77 Hz, 85 Hz, 93 Hz, 101 Hz in left ear and 79 Hz, 87 Hz,95 Hz, 103 Hz in right ear, were presented simul-taneously by the insert phones. All the subjectswere sedated by Hydrate Chloride during ASSRtesting. Children aged 6 months to 24 months oldwere tested with visual reinforce audiometry and25 months to 5 years old were with playaudiometry. The thresholds of ASSR at each car-rier frequency were compared to the correspond-ing behavioural thresholds. RESULTS①ASSRthresholds were usually above behaviouralthresholds. The mean and standard deviation ofthe difference at 0.5kHz, 1kHz, 2kHz, 4kHz were15.2±9.1dB, 8.8 ±7.5dB, 8.2±7.7dB and 10.7±8.8dB respectively. ANOVA showed significantmain effect between carrier frequencies on thedifference between ASSR thresholds andbehavioural thresholds(F=10.395; μ 3 ; P=0.000).Post Hoc analysis indicated that this effect wasdue to the highest threshold found at 0.5kHz.②The behavioural thresholds and the responsethresholds at 0.5kHz, 1kHz, 2kHz, 4kHz were sig-nificantly correlated and the correlation coeffient(r) was 0.799,0.859,0.894,0.850 respectively.CONCLUSION Being objective, frequency-spe-cific and well correlated with behavioral thresholds,ASSR to multiple simultaneous stimuli is provento be an alternative of behavioral hearing test fordifficult-to-test children and those whose hearingacquired by behavioral assessment were non-ear-specific.[
