摘要
目的 探讨急性脑出血患者血清白介素 6 (IL 6 )和转化生长因子 β1(TGF β1)水平在发病过程中的动态变化及其临床意义。方法 采用双抗体夹心法检测 30名正常人 (对照组 )和 30例急性脑出血患者病后第 1、3、7、14d的血清IL 6和TGF β1浓度。结果 脑出血组在病程第 1、3、7d血清IL 6水平均比对照组明显增高 (均P <0 0 5 ) ,TGF β1水平均明显减低 (均P <0 0 5 ) ,至第 14d血清IL 6、TGF β1水平均接近正常水平。血清IL 6水平在病程第 1d达到峰值 (P <0 0 5 ) ,TGF β1在病程第 3d为低谷值 (P <0 0 5 )。大量脑出血患者血清IL 6水平均明显高于中、小量出血患者 (均P <0 0 5 )。不同出血量患者之间TGF β1水平的差异无显著性(P >0 0 5 )。神经功能缺损程度与血清IL 6水平呈正相关 (r=0 6 2 5 ,P <0 0 5 ) ,与TGF β1水平无关 (r=0 0 0 1,P >0 0 5 )。IL 6水平与TGF β1水平无明显相关性。结论 IL 6、TGF β1与脑出血的病理过程有密切关系。血清IL 6浓度测定对判断脑出血的病情严重程度有一定意义。
Objective To investigate the changes and clinical significance of serum interleukin-6(IL-6) and transforming growth factor-β 1(TGF-β 1) levels in patients with acute cerebral hemorrhage.Methods The levels of serum IL-6 and TGF-β 1 were serially measured(1,3,7,14 d) by enzyme-linked immunosorbent assay(ELISA) method in 30 patients with acute cerebral hemorrhage and 30 healthy controls.Results At day 1, 3, and 7, the levels of IL-6 in patient group were significantly higher than those in control group(all P< 0.05), but the levels of TGF-β 1 were significantly decreased(all P< 0.05). At day 14, the IL-6 and TGF-β 1 levels returned to normal value. In patient group, IL-6 level got the peak at day 1( P<0.05) and TGF-β 1 reached the nadir at day 3( P< 0.05). The IL-6 levels in patients with large volume of hematoma were obviously higher than those in patients with moderate or small volume of hematoma (all P< 0.05). But no remarkable difference of TGF-β 1 level was found among the patients with different volume of hematoma( P> 0.05). The degree of neurological deficits was positively correlated with IL-6 level( r= 0.625, P< 0.05) and was not correlated with TGF-β 1 level( r= 0.001, P> 0.05). Furthermore, no correlation was found between IL-6 level and TGF-β 1 level.Conclusions IL-6 and TGF-β 1 play important role in the pathophysiology of cerebral hemorrhage. IL-6 may be of value in predicting the severity of the disease.
