摘要
本文报告区域性门静脉高压症5例,术前均误诊。其临床表现与肝内型门静脉高压症相似,常有胰腺病史和慢性胰腺炎的表现,如左上腹痛及包块等。B 超检查和经肝、脾静脉造影可协助诊断。术中可见脾胃静脉曲张明显而肝外观正常,应注意探查胰腺和脾静脉(有无血栓形成),测门静脉压力。
In this paper,5 cases of regional portalhypertension
were reported,and all were misdiagnosed preoperatively.
Clinical manifestations of this disease were previous simi-
lar to these of intrahepatic type portalhypertension,usual-
ly with a history of pancreatic disease and features of
chronic pancreatitis such as mass and pain at left upper
abdomen.The use of B type ultrasonography and tran-
shepatic or transsplenic veinography were helpful to the
diagnosis.Marked varicoses of the Spleno—gastric veins
and normal appearances of the liver were the main find-
ings in operation.During the operation it is important to
explore specifically the pancreas and the splenic vein (to
detect the presence of thrombosis)as well as to measure
the portal vein pressure.
