摘要
目的 :研究抗凋亡基因bcl- 2和促凋亡基因bax在胃癌发生、发展过程中的变化。方法 :应用SP免疫组织化学方法 ,对 80例胃癌及癌旁粘膜组织进行bcl- 2、bax基因蛋白的检测。结果 :胃癌组织中bcl- 2阳性表达率为 4 6 .3%,癌旁组织为 4 0 .0 %。光镜下对癌旁粘膜组织依有无癌前病变进行分组 ,其中 5 6例伴癌前病变者bcl - 2阳性表达率为 5 7.1 %,2 4例不伴癌前病变者无阳性表达。对 5 3例带有癌的癌旁组织根据癌旁粘膜和癌的关系 ,分为移行粘膜 2 5例 ,受侵粘膜 2 8例 ,其中移行粘膜均伴癌前病变。bcl - 2阳性表达率在移行粘膜(6 8.0 %)和胃癌组织均明显高于受侵粘膜 (3.5 7%)。bax阳性表达率在胃癌为 4 1 .3%,癌旁组织为 32 .5 %。伴癌前病变癌旁粘膜 (1 4 .2 %)和胃癌组织及不伴癌前病变癌旁粘膜(75 .0 %)均有明显差异。本组实验中 ,bcl- 2与bax共同阳性表达率在伴癌前病变癌旁粘膜(1 2 .5 %)和胃癌 (1 3.8%)明显高于不伴癌前病变癌旁粘膜 (0 ) ;移行粘膜 (2 8.0 %)明显高于受侵粘膜 (3.5 7%)。且胃癌、伴癌前病变癌旁粘膜、不伴癌前病变癌旁粘膜 ;胃癌、移行粘膜、受侵粘膜组间均有明显差异。结论 :bcl- 2、bax基因蛋白的异常表达与胃癌发生有关 ;在伴癌前病变癌旁粘膜组织中bcl- 2表达明显高于bax。
Objective:To study the changes of bcl-2 and bax oncoprotein in the occurrence and development of gastric carcinoma (GC).Methods:The cellular oncoprotein expressions of bcl -2 and bax were studied with streptavidin peroxidase immunohistochemical method . Specimens taken from 80 gastric carcinoma(GC) and mucosa tissues adjacent to the primary cancer (MA) were examined in this series. Results: The positive rates of bcl-2 oncoprotein were 46.3%, 40.0% in GC and MA. MA was further devided into two groups on the basis of precancerous lesions (PL) under light microscope.The positive expression rate of bcl-2 was 57.1% for the 56 cases MA with PL( MAWPL) , while no positive expression of bcl-2 was found in the other 24 cases MA without PL( MAWOPL) .According to the relationship between MA and GC,53 cases MA with cancer were subdivided into transitional mucosa (TM,25 cases) and invaded mucosa (IM,28 cases).All the TM were with PL.The rate of expression of bcl-2 in TM (68.0%) and GC was significantly higher than that in IM(3.57%). The positive rates of bax oncoprotein were 1.3%,32.5% in GC and MA .The bax expression in GC and in MAWOPL (75.0%)was significantly higher than that in MAWPL(14.2%). In this study,the coexpression rates of bcl-2 and bax oncoproteins were significantly different among MAWPL (12.5%) and MAWOPL (0%),GC(13.8%)and MAWOPL, TM (28.0%) and IM(3.57%), in GC,MAWPL and MAWOPL, and on GC,TM and IM. Conclusion: the abnormal expressions of bcl-2 and bax in GC were related to generation of GC. The expression of bcl-2 gene was significantly higher than that of bax gene in MAWPL. [
