CT与静脉肾盂造影在肾结核诊断中的比较 被引量:4

摘要 目的 :通过研究肾脏结核在CT、IVP两种影像中的表现与病理所见的关系 ,来揭示肾结核的影像学表现在二种检查方法之间的互补性 ,以期达到早发现、早确诊、早治疗的目的。方法 :搜集 130例经病理证实为肾结核的病例 ,回顾性总结 ,对CT平扫及增强、IVP二种检查方法进行比较。结果 :CT诊断符合率 93.0 % ,不能明确诊断 7% ,IVP诊断符合率 36 .6 % ,未能明确诊断 6 3.4 %。结论 :CT能清楚显示整个肾脏的横断面图像 ,对肾实质及肾盂、肾盏的形态结构均显示清楚 ,且有很高的密度分辨率 ,对空洞及肾内钙化的检出率明显高于平片及肾盂造影。增强扫描及延迟扫描可以用于肾功能的判断、与肾盂旁囊肿、肾盂肿瘤的鉴别。所以肾结核的诊断 ,CT明显优于肾盂造影。 Objective: By studying the relationship between manifestation of Renal tuberculosis in two ways of CT and VIP and the characters of its pathology, we tried to reveal the complementation of the two ways in image manifestation of Renal tuberculosis in order to attain the aim of early detecting ,early diagnosing and early treating . Methods: Collecting 130 cases of pathologically-confirmed Renal tuberculosis, we made retrospective summary and comparison between CT plain scanning and IVP. Results: The diagnostic rate for tuberculosis by CT was 93.0%, the undiagnostic rate by CT was 7%, the corresponding figures by VIP were 36.6% and 63.4% respectively. Conclusion: CT can clearly reveal the whole Kidney in transect-image ,shape structure of Renal parenchyma and Renal calyx with higher density resolving power, and the detecting rate of cavitas and renal calcification was significantly higher than those of X-ray and IVP. Enhancing and delaying scanning can apply to the judgement of Renal function and identification of the cyst besides the Renal calyx and Renal tumors . Therefore ,CT is significantly higher than IVP in Renal tuberculosis diagnosis rate.
