血供重建与骨重建对股骨头缺血坏死兔骨细胞发育的影响(英文)

Effects of revascularization and bone remodeling on the development of bone cells in rabbits with avascular necrosis of femoral head

摘要 背景:股骨头缺血坏死的致病原因已得到确定,治疗的术式有多种,但哪种方法可加快骨修复速度,使骨组织在较短时间内得以重建的效果更为理想?目的:建立股骨头缺血坏死动物模型,寻找治疗此类疾病的有效方法以更好的促进骨细胞的发育。设计:随机对照实验研究。单位:哈尔滨医科大学第二附属医院骨科。材料:实验在哈尔滨医科大学第二附属医院骨科实验室完成。普通级健康大耳白兔27只,雄性,4个月龄。方法:将股骨头离断后埋于皮下组织中,4周后分成3组:A组为无处置对照组,B组使用股深动脉植入治疗,C组使用植入血管加游离骨膜治疗。对各组的治疗效果进行对照分析。主要观察指标:①大体观察。②光镜观察。③电镜观察。结果:B,C组骨小梁间隙网状组织内可见红细胞充满,有明确的血供重建,而C组骨小梁间隙内有大量的骨髓细胞出现,电镜下骨基质排列整齐,有周期性横纹,骨细胞发育正常。结论:截断股骨头皮下埋藏是制作股骨头缺血坏死动物模型的有效方法,所采用的两种方法对改善缺血坏死,促进骨细胞的发育均有一定效果,以C组的方法最佳。 BACKGROUND:The causes of avascular necrosis of femoral head have been confirmed and there are various kinds of therapeutic methods for it. However,it is still uncertain that which method will be better for quicker recovery. OBJECTIVE:To build animal models of avascular necrosis of femoral head in order to find an effective method to better stimulate the development of bone cells. DESIGN:A randomized controlled trial. SETTING:Department of Orthopaedics,the Second Affiliated Hospital of Harbin Medical University. MATERIALS:This experiment was completed in the Laboratory of Orthopaedics,the Second Affiliated Hospital of Harbin Medical University. Twenty-seven common class healthy white flap-eared rabbits of 4 months old were chosen. METHODS:Femoral head was removed and embedded into endermic tissues of rabbits.Four weeks later,the rabbits were divided into three groups:group A(control group),group B(implanted deep femoral artery as treatment), group C(implanted blood vessel and free periosteum as treatment).Comparative analysis was conducted for the therapeutic effects among groups. MAIN OUTCOME MEASURES:①Gross observation;②Light microscope observation;③Electron microscope observation RESULTS:In groups B and C,the reticular tissues between bone trabeculas were full of red cells,indicating that there was obvious revascularization.However,in group C,there were plenty of marrow cells appeared in the clearances of bone trabeculas.The bone matrix arranged orderly under the observation of electronic microscope with periodically horizontal lines and the bone cells were in normal development. CONCLUSION:Amputating femoral head and embedding into endermic tissues is an effective method to establish animal model of avascular necrosis of femoral head.All the two methods adopted can improve the avascular necrosis and accelerate the development of bone cells,of which the method for group C is the better.
