幕上星形胶质细胞瘤DNA含量与磁共振征象的相关性研究 被引量:1

摘要 目的比较星形胶质细胞瘤磁共振(MR)征象与肿瘤细胞核DNA含量的相关性,从核磁共振成像(MRI)影像学角度评价星形胶质细胞瘤的恶性生物学行为。方法应用流式细胞技术检测55例星形胶质细胞瘤肿瘤组织DNA含量及细胞增生水平,计算出肿瘤DNA指数(DNAindex,DI)和细胞增生指数(proliferationindex,PI),将MR征象与DNA指数和细胞增生指数进行对照研究。结果星形胶质细胞瘤DI值为1.23±0.22,PI值为38±3。DI、PI值随病理级别的增加逐渐增高,差异有显著性(P<0.01)。MR图像上,与DI、PI关系密切的征象是囊变坏死、信号不均匀、出血、边缘不清楚、增强程度(P<0.05或P<0.01)。结论星形胶质细胞瘤MR征象与DNA含量、细胞增生关系密切。MR图像上,肿瘤囊变坏死、信号不均匀、出血、边缘模糊、强化明显者,其DNA含量较高,可作为判断肿瘤恶性生物学行为的影像学指标。 Objective To study the correlation of magnetic resonance imaging (MRI) features with the tumor nuclear DNA contents, and evaluate the biological behavior of malignant astrogliocytomas using MRI. Methods The measurment of DNA contents and cell proliferation was performed on 55 cases of supratentorial astrogliocytomas all by flow cytometry (FCM). The correlation of MRI features with DNA index (DI) and proliferation index (PI) was studied. Results In 55 cases of astrogliocytomas, DI was 1.23±0.22, PI 37.91±3.37. As the pathologic grade increased, DI and PI were also gradually increased in low-grade astrocytoma, anaplastic astrocytome and glioblastoma multicforme, there was significant difference among the three groups (P<0.01). DI and PI had correlation with cyst degeneration or necrosis, signal heterogeneity, hemorrhage, unclear border definition, and contrast enhancement degree demonstrated on MRI images (P<0.05, P<0.01). Conclusion MRI features closely correlated with DNA contents and cell proliferation in astrogliocytomas. DNA contents were higher in the tumor with cyst degengration or necrosis, signal heterogeneity, hemorrage, unclear border definition, marked contrast enhancement. All of these imaging appearances can be employed to estimate the biological behavior of the malignant tumor.
