摘要
目的 旨在了解北京市城区老年人膝关节骨关节炎 (OA)的患病率 ,并比较北京市城区老年人膝关节炎的患病率同美国白人患病率的差异。方法 从北京市 3个城区 6 0岁以上的北京男女居民随机抽取样本 ,其中男 730例 ,女 1 0 5 1例。所采用研究方案与美国Framingham骨关节研究方案一致。受检者分别完成入户调查及其问卷 ,然后受检者到医院进行双膝关节持重正位X线检查。采用年龄标化患病率比表达该研究和美国Framingham研究中膝关节OA患病率的差异。结果膝关节X线OA患病率女性为 4 2 8%,男性为 2 1 5 %;临床OA患病率女性为 1 5 0 %,男性为 5 6 %。同美国Framingham研究的白人OA患病率比较 ,女性膝关节X线和临床OA患病率均高于美国同龄白种人 ;男性膝关节X线和临床OA患病率与美国同龄白种人相似。结论 北京市城区老年女性膝关节炎患病率较高 ,且高于美国同龄妇女的OA患病率 ,应引起重视。
Objective To investigate the prevalence of elderly knee osteoarthritis (OA) in Beijing and to compare it with elderly whites in the United States.Methods A random sample of persons with age more than 60 years (730 males and 1051 females) from 3 central districts of Beijing was recruited in the study.Protocol of our study was identical to that of Framingham OA study in United States.All subjects were interviewed and completed questionnaires which including knee symptoms etc.Then they went to hospital to have weight-bearing bilateral knee radiographs.The prevalence of the OA in elderly subjects in Beijing was estimated and compared with OA prevalence in Framingham, using an age-standardized prevalence ratio.Results The prevalence of radiographic knee OA was 42.8% in women and 21.5% in men.Symptomatic knee OA was 15.0 % in women and 5.6% in men.Women in Beijing had a higher prevalence of both radiographic and symptomatic OA when comparing with the same age whites in Framingham.The prevalence of both radiographic and symptomatic OA in Beijing men was similar to that in white men in Franmingham.Conclusion Noteworthiness from the study is that elderly women in Beijing have higher prevalence of OA than that of white women in the United States.
