摘要 目的 探讨“VanPraagh节段分析法”在实时三维超声心动图技术 (Real -timeThree -DimensionalEchocardio graphy ,RT 3DE)诊断复杂型先心病中的应用价值。 方法 各种复杂型先心病 57例 ,年龄 1 4d~ 1 4岁 1 1个月 ,平均 (2 .78±2 .94)岁 ,分别应用节段分析法进行RT 3DE及二维彩色多普勒超声心动图 (CDE)诊断 ,并与手术和 (或 )心血管造影结果进行对照。结果 RT 3DE补充或纠正CDE诊断 6例 (1 0 .53 % ) ,所在节段分别为心室位置 2例 ,心室 -大动脉连接关系 1例 ,判断合并室间隔缺损和缺损定位 2例 ,瓣膜病变类型 1例。结论 RT 3DE结合“VanPraagh节段分析法”为超声诊断复杂型先天型心脏病提供更准确的方法 ,尤其在判断心室位置、心室 -大动脉连接关系、病变类型等方面能提供更准确的空间解剖信息。 Objective To evaluate van praagh segmental analysis in the diagnosis of complex congenital heart diseases using real-time three-dimensional echocardiography (RT-3DE).Methods Fifty seven pediatric patients (male 38, female 19) with complex congenital heart diseases, aged 2.78 -2.94 years, were examined by RT-3DE and two-dimensional color doppler echocardiography (CDE) with Philips/SONOS7500 ultrasonic system. Van Praagh segmental analysis was used in the process of diagnosis, which was compared with angiography and/or surgical findings.Results Based on the angiography and/or surgical findings, diagnoses made by CDE were corrected, revealed, or modified by RT-3DE in 6 patients (10.53%), which included the position of ventricles (2 cases), ventricular-arterial connection (1 case), type and location of ventricular septal defects (2 cases) and type of valvular lesion (1 case).Conclusions RT-3DE with Van Praagh segmental analysis is more accurate for the diagnosis of complex congenital heart diseases, especially in identifying the position of ventricles, the ventricular-arterial connection,type and location of the lesions.
