摘要
目的 :制作肠道外引流的异基因大鼠胰、脾、十二指肠移植模型 ,观察供体脾脏对胰腺移植的影响 ,并改进部分手术技术。 方法 :封闭群雌性SD大鼠为供体 ,封闭群雄性Wistar大鼠为受体。用一定点连续缝合法吻合血管、腰部侧后腹膜外注入肝素、供体十二指肠腹腔外引流等方法改进模型制作技术。 结果 :制作模型 2 1只 ,17只 (80 .9% )存活超过 7d。供体手术耗时 (5 1± 6 )min。受体手术耗时 (5 8± 7)min ,其中静脉吻合平均 8min ,动脉吻合平均 12min ,肠道处理平均 6min。死亡 4只。死因分别为 :供体十二指肠腔内出血、腹腔内出血和受体腹主动脉腔内血栓形成。结论 :手术技术的改进能有效地减少模型术后并发症的发生 ,提高模型的存活率。
Objective: To establish the model of pancreaticodu odenal and splenic transplantation in the allogenetic rats for investigating the effect of grafted spleen upon pancreaticoduodenal transplantation and to improve the technology of microsurgery. Methods: The female SD rats served as donors and the male Wistar rats served as recipients. The means of one point fix continuous suture was used to anastomose vascular. Heparins were used by injecting to outside peritoneum of waist. External drainage of donor's duodenum was made. Results: Total 21 models of rat were performed. The surviving time of 17 models were longer than 7 d,the success rate was 80.9%. The spent time of operation on donor and on recipient was separately (51±6) min and (58±7) min. The mean time was 8 min spent on vein anastomosis, 12 min on aorta anastomosis and was 6 min on donor's duodenum processing. 4 rats died. Lethal cause included hemorrhage in donor's duodenal cavity and in abdominal cavity, thrombosis in recipient's abdominal aorta. Conclusions: The technical improvement can decrease the postoperative complications of models and increase the model's livability.
