口服乙酰唑胺负荷试验单光子发射计算机断层扫描对短暂性脑缺血发作患者的早期诊断价值 被引量:3

摘要 为评价口服乙酰唑胺 (ACZ)血管激活试验单光子发射计算机断层扫描 (SPECT)对短暂性脑缺血发作(TIA)患者的诊断价值。方法 对 10名正常志愿者和 30例TIA患者进行静息和口服ACZ血管激活试验的SPECT显像 ,并进行断层图像的视觉及半定量分析。观察口服ACZ前后血气分析变化。结果 正常ACZ激活试验后 ,99mTc ECD在大脑两侧对应部位呈对称性分布 ,脑血流量普遍增加 ,TIA组ACZ血管激活试验后 ,血pH值降低 ,二氧化碳分压 (PCO2 )增高。发作期TIA患者SPECT显像呈现局部低灌注的患者由 5例 (5 6 )增加至 6例 (6 6 ) ,静止期患者的SPECT阳性率由 6 0 %提高至 92 % ,ACZ激活试验后总阳性率为 93%。结论 口服ACZ血管激活试验方法简单可靠 ,对TIA患者早期的影像学诊断有重要价值。 Objective To assess the diagnostic value of acetazolamide(ACZ) vasoactive test using regional cerebral blood flow (rCBF) SPECT imaging in patients with TIA.Methods SPECT imaging was performed before and after oral ACZ vasoactive test combining with visual and semiquantitative analysis of tomogram.Blood gas analysis was also done before and after ACZ test.Results After ACZ test in normal group, 99m TC ECD was distributed symmetrically on corresponding parts of the brain and CBF was generally increased.The value of blood pH was decreased and blood PCO 2 was increased in TIA group after ACZ test.The positive rate of local hypoperfusion lesion on SPECT imaging increased from 5/6 to 6/6 in symptomatic TIA patients and from 60% to 92% in asymptomatic patients.The total positive rate was 93% after ACZ test.Conclutions The method of oral ACZ vasoactive test using SPECT imaging was simple,reliable and has early diagnostic value in patients with TIA.
