摘要
目的 探讨汞 (Hg)、镉 (Cd)、砷 (As)、铅 (Pb)与人群中多指 (趾 )畸形的关系。 方法 采用 1∶1配对病例对照研究方法 ,在“中美预防出生缺陷监和残疾合作项目”监测系统中 ,募集新近发生的 86对多指 (趾 )畸形病例和相应对照 ,问卷调查引起出生缺陷可能的环境危险因素的暴露情况 ;采用微波消化、ICP质谱分析等方法 ,测定了多指 (趾 )畸形病例和对照组产妇早孕阶段毛发中汞、铅、砷、镉元素的含量 ;利用COX比例风险模型进行流行病学配对资料多因素分析。 结果 孕期从事化工职业工作、膳食中水果类摄入较少以及发汞含量为 3个有显著性意义的环境危险因素 ,对应的OR值分别是 9 99、 1 89和 1 95 ,P值分别是 0 0 2 9、 0 0 2 8和 0 0 2 2。 结论 孕期工作环境、饮食情况与胎儿多指畸形有关 ;孕早期汞的环境暴露可能是多指 (趾 )畸形的危险因素之一。
ObjectiveTo explore the potential relationship between some toxic elements such as mercury,cadmium,arsenic and lead,and human polydactyly.MethodsBy condncting an 1∶1 matched case control study,we collected 86 pairs of polydactyly cases and controls from the Sino O.S Collaborative Project on Neural Tube Defects Prevention and Birth Defects Surveillance System.Environmental risk factors were obtained from field investigation with questionnaires;Hair elements that grown out during the first three months of pregnancy were detected by microwave digestion method and ICP MS methods.Cox Proportional Hazard Regression Model was used to perform the multi variable conditional Logistic regression analysis.ResultsOccupation of a chemical plant,low level fruits intake,and higher hair mercury level are three significant risk factors included in the last Cox Regression Mode.The OR values are 9 99,1 89 and 1 95,respectively,and the corresponding P value are 0 029,0 028 and 0 022,respectively.ConclusionOccupational environments and pregnancy of diet habits are related to fetus polydactyly.Mercury exposure during the first three months may be a new environmental risk factor for human polydactyly.
