摘要
目的探讨瘦素与围绝经期妇女肥胖及脂类代谢的关系。方法分别测量了72例围绝经期妇女(研究组)血清瘦素(leptin)、胰岛素(INS)、低密度脂蛋白胆固醇(LDLC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDLC)、载脂蛋白A1(apoA1)、载脂蛋白B(apoB)、垂体促性腺激素(LH、FSH)雌二醇(E2)、睾酮(T)、孕酮(P)垂体泌乳激素(PRL)值及体重、身高,与50名正常育龄妇女(对照组)进行对照分析。结果围绝经期肥胖组、非肥胖组瘦素、胰岛素水平增高与对照组比较差异有显著意义(P<001)。肥胖组LDLC494mmol/L、TG21mmol/L、apoB0865g/L与非肥胖组及对照组比较差异有显著意义(P<001)。研究组内LH、FSH、E2、T值,差异均无显著意义(P>005),但与对照组相比差异有显著意义(P<001)。相关分析显示围绝经妇女瘦素与BMI、胰岛素、LDLC、TG、apoB高度相关(r=0762,P=00002;r=0382,P=001,r=0430,P=0002;r=0545,P=00002;r=0454,P=00002),且LDLC与apoB同步升高呈相关(r=0796,P=00002)。围绝经期瘦素水平与HDLC、apoA1分析未见有相关(r=-0222~0190,P>005)。结论围绝经期瘦素水平与脂类代谢异常相关,肥胖、瘦素增高围绝经期妇女更应重视脂类代谢异常。
Objective To discuss the functions of hormones relative to metabolism ,such as leptin and insulin on obesity and lipids metabolism of climacteric women after examining the variational levels of these hormones Methods Choose 72 cases of climacteric women (research group) and 50 cases of normal reproduction age women (control group), determine the values of following indexes and draw an contrast analysis The values are: leptin, insulin(INS), low density lipoprotein cholesterol(LDL C), triglycerides(TG), high density lipoprotein cholesterol (HDL C), apolipoprotein AI(apoAI), apolipoprotein B (apoB) serum pituitary gonadotropin (LH,FSH), estradiol(E 2), testosterone(T), progesterone(P), prolactin(PRL) in serum of both groups Results The enhance of leptin and INS in both the obese group and non obese group orf research group have marked differences with control group( P <0 01) There are obvious differences among the obese group(LDL C4 94mmol/L,TG2 1mmol/L,apoB 0 865g/L),non obese group and control group too( P < 0 01) The values of LH,FSH, E 2 and T in research group have no noticeable differences( P > 0 05), but bear striking differences to control group ( P < 0 01) The relativity analysis show that: the level of leptin in the climacteric women is positively relative to BMI, insulin,LDL C,TG and apoB ( r = 0 762, P = 0 000 2; r = 0 382, P = 0 01; r =0 430, P = 0 00 2; r = 0 545, P = 0 000 2; r = 0 454, P = 0 000 2) Moreover, LDL C is markedly correlative to the synchronological increase of apoB There are no correlation among leptin, HDL C and apoAI( r = -0 222 ~0 190 , P > 0 05) Conclusions The climacteric level of leptin is correlative to the abnormal metabolism of lipids, Those climacteric women with obesity and leptin increase should pay keen attention to the abnormal metabolism of lipids
