早期激素性股骨头缺血坏死MR灌注及病理相关性实验研究 被引量:3

摘要 目的:分析激素性股骨头缺血坏死(ANFH)模型不同时期的股骨头MR灌注情况,并与病理学结果相对照。方法:20只健康日本大耳白兔,分别于给药前和给药后2周、4周、6周、8周以相同方法行MR常规及Gd-DTPA灌注增强扫描,绘出股骨头的时间-信号强度变化动态曲线,分析股骨头不同阶段骨髓信号强度变化的情况。处死后取材采用HE染色光镜观察及电镜超微结构分析。结果:早期激素性ANFH血流灌注初期时间-信号强度变化曲线的斜率逐渐减小,且灌注的强度逐渐下降(P<0.01)。相应的骨小梁空虚骨陷窝数逐渐增多(P<0.01)。结论:股骨头部各种细胞的脂肪变性与坏死是激素性ANFH的早期病理改变,应用快速MR动态增强技术能够检查股骨头的血流灌注情况,二者有显著相关。 Objective To analyze marrow perfusion of the model of steroid induced ANFH and invesigate the corresponding pathological changes in the femoral head of different stage of ischemia. Materials and Methods Twenty Japanese white rabbits were examined by MR in the same method before treatment and in the second week fourth week sixth week and eighth week after treatment. Time signal intensity dynamic curve was depicted to analyze the marrow signal intensity changes of femoral head in different stage of ischemia. After rabbits were killed sample were examined histologically. HE stained specimen and EM ultramicrostructure were analyzed. Results The slope of time signal intensity curve gradually declined in early stage of blood perfusion and intensity of perfusion decreased gradually(P<0.01). Correspondlingly the number of empty lacuna increased gradually(P<0.01). Conclusion The main pathological changes of steroid induced ANFH in the early stage were fat degeneration and necrosis of various kinds of cells in femoral head. Dynamic enhanced technique can evaluate the state of blood perfusion of femoral head.
