摘要
目的探讨仰、俯卧位对新生儿肺炎通换气功能的影响.方法于2003年1~10月,将中国医科大学二院NICU住院的18例肺炎早产儿和14例肺炎足月儿分别置于仰卧位和俯卧位,分别测定2种体位时吸入氧体积分数(FiO2)、呼吸频率(RR)、动脉血氧分压(PaO2)和动脉血二氧化碳分压(PaCO2),并计算出动脉血氧分压与吸入氧体积分数的比值(PaO2/FiO2)、动脉血氧分压与肺泡气氧分压的比值(PaO2/PAO2)及肺泡动脉氧分压差(A-aDO2).结果早产组及足月组肺炎患儿俯卧位时PaO2、PaO2/FiO2及PaO2/PAO2明显高于仰卧位(P<0.05),A-aDO2明显低于仰卧位(P<0.05).两组肺炎患儿仰、俯卧位时RR及 PaCO2的改变均无统计学意义(P>0.05).结论俯卧位可改善早产儿和足月儿肺炎的氧合,这提示俯卧位可能为改善新生儿肺炎患儿氧合功能的适宜体位.
Objective To evaluate the effect of supine and prone position on ventilation and gas exchange of preterm and term infants with pneumonia.Methods 18 preterm infants and 14 term infants with pneumonia were studied in both supine and prone positions.In the two positions,fraction of inspired oxygen (FiO 2)、respiratory rate (RR)、arterial oxygen tension (PaO 2) and arterial carbon dioxide tension(PaCO 2)were determined,then arterial oxygen tension/fraction of inspired oxygen(PaO 2/FiO 2)、arterial oxygen tension/alveolar oxygen tension(PaO 2/P AO 2)and alveolar-arterial oxygen tension gradient(A-aDO 2) were obtained.Results In the preterm and term infants with pneumonia,PaO 2、PaO 2/FiO 2 and PaO 2/P AO 2 in the prone position were signicantly higher than in the supine position(P<0.05),A-aDO 2in the prone position were signicantly lower than in the supine position(P<0.05).In the two groups,the change of RR and PaCO 2 in the two positions had no statistical difference (P>0.05).Conclusion Oxygenation of both preterm and term infants with pneumonia could be improved in prone position which might be a better position.
