摘要
目的 评价心肌造影实时三维超声心动图(RT 3DE)测量缺血心肌质量的可重复性与准确性。方法 对21只健康杂种犬行开胸手术,结扎冠状动脉分支造成心肌缺血,分别于结扎前后进行 RT 3DE检查。静脉注射氟烷造影剂进行心肌造影,勾划相应的缺血心肌体积并计算其质量。结果 心肌造影 RT 3DE所测缺血心肌质量为(12.3±5.3)g,与伊文思蓝染色后离体测量结果显著相关( r=0.96)。两种测量方法的组间及组内差异比较具有高度的一致性。结论 心肌造影RT 3DE能准确计算犬缺血心肌质量,有望为临床定量评价冠心病患者心肌缺血提供一项新的手段。
Objective To study the accuracy and reproducibility of measuring myocardial perfusion defects with intravenous contrast enhanced real-time three-dimensional echocardiography(RT-3DE).Methods Myocardial contrasted RT-3DE was performed in 21 open-chest mongrel healthy dogs after acute ligation of left anterior descending coronary artery (LAD,n=14) or distal branch of the left circumflex artery (LCX,n=7).A perfluorocarbon microbubble contrast agent was injected intravenously to assess the resulting myocardial perfusion defect with a commercially available Philips Sonos 7500 ultrasound system.Blinded off-line calculation of left ventricular(LV) mass and perfusion defect mass from RT-3DE images was performed using an interactive aided-manual tracing technique.Evans blue dye was injected into the occluded coronary artery for subsequent anatomic identification of under-perfused myocardium.Results Total LV mass was ( 60.0± 10.1)g.The perfusion defect mass was ( 12.0± 5.0)g.The myocardial contrasted RT-3DE estimation of total LV mass [( 59.8± 9.9)g] strongly correlated with the anatomic measurements (r= 0.98).The myocardial contrasted RT-3DE calculation of the under-perfused myocardial mass [( 12.3± 5.3)g] also strongly correlated with the ex vivo raw measurements (r= 0.96).Conclusions RT-3DE with myocardial contrast opacification provides an accurate measurement of perfusion defects in an animal model of coronary ligation.This novel imaging modality may play an important role in quantitative assessment of myocardium at risk in the patients with coronary artery disease.
