摘要
【目的】研究胰腺癌和慢性胰腺炎EMMPRIN ,MMP1,MMP9,TIMP1表达及临床意义 ,并探讨其在胰腺癌中表达的相互关系。【方法】5 1例胰腺癌和 10例慢性胰腺炎手术切除标本常规制作石蜡包埋切片 ,上述调控蛋白表达的染色方法均为ABC免疫组化法。【结果】胰腺癌EMMPRIN ,MMP1和MMP9表达阳性率及其评分明显高于慢性胰腺炎 (P <0 .0 1) ,但TIMP1则相反 (P <0 .0 1)。高分化腺癌和无转移癌EMM PRIN、MMP1和MMP9表达阳性率及其评分明显低于低分化腺癌和转移癌 (P <0 .0 5或P <0 .0 1) ,TIMP1则相反 (P <0 .0 5或P <0 .0 1)。EMMPRIN、MMP1和MMP9在胰腺癌中表达评分值之间呈密切正相关 (P<0 .0 1) ,但均与TIMP1呈高度负相关 (P <0 .0 1)。【结论】EMMPRIN、MMP1、MMP9和TIMP1表达可反映胰腺癌发生发展、生物学行为、侵袭或转移潜能及预后 ,均为重要生物学标记。EMMPRIN诱导胰腺癌细胞产生MMP1和MMP9;但抑制癌细胞产生TIMP1,同样TIMP1能抑制MMP1和MMP9的活性表达。
Objectives]To study the expression of extracellular matrix metalloproteinase inducer(EMMPRIN),matrix metalloproteinaxe-1(MMP 1), matrix metalloproteinaxe-9(MMP 9) and tissue inhibitor of metalloproteinase-1(TIMP 1) and its clinicopathological significances in pancreatic carcinoma (PC) and chronic pancreatitis (CPT) and to explore their mutual relationships in pancreatic carcinoma.Immunohistochemical method of avidin-biotin complex was used for detecting expression levels of EMMPRIN, MMP 9, TIMP 1 on the routinely paraffin-embedded sections of surgically resected specimens from 51 cases of PC and 10 cases of CPT. The positive rates and scores of EMMPRIN, MMP 9 and MMP 1 in PC were significantly higher than those in CPT(P<0.01), but those of TIMP 1 were just opposite(P<0.01). The positive rates and scores of EMMPRIN, MMP 1 and MMP 9 in well-differentiated adenocarcinoma and cancer without metastasis were significantly lower than those in poorly differentiated adenocarcinoma and metastatic cancer (P<0.05 or P<0.01), but those of TIMP 1 were converse (P<0.05 or P<0.01). In PC tissues, close positive correlations existed among the scores of EMMPRIN and those of MMP 1(r=0.825) and MMP 9(r= 0.513)(P<0.01); whereas highly negative correlations were found among the scores of TIMP 1 and those of EMMPRIN(r= -0.513), MMP 1(r=-0.474) and MMP 9(r=-0.653)(P<0.01).[Conclusion]The expression of EMMPRIN, MMP 1, MMP 9, and TIMP 1 may serve as important biological markers for reflection of carcinogenesis, progression, biological behaviors, invasive potential, metastatic status and prognosis in pancreatic carcinoma. EMMPRIN induces the PC cells to produce MMP 1 / MMP 9 and inhibits their TIMP 1 production. TIMP 1 also possibly inhibits the active expressions of MMP 1 and MMP 9 in cancer cells.
