Effects of cell-cycle-related factors on biological behavior and evaluation for prognosis of breast carcinoma

摘要 目的:研究乳腺癌组织中细胞周期蛋白D2、细胞周期蛋白依赖性激酶4(cyclin-dependentkinase4,CDK4),探讨它们与P27kip1蛋白表达、与临床病理指标的关系及其预后意义。方法:对1996-01/2000-12在河北工程学院医学院病理教研室进行病理检查的乳腺癌标本用免疫组化S-P法,检测96例乳腺癌,18例癌旁正常乳腺组织石蜡切片中CyclinD2和CDK4的表达,比较它们与P27kip1蛋白表达,与临床病理学指标之间的关系。结果:①96例乳腺癌组织中CyclinD2,CDK4和P27kip1的表达率分别为42%,54%和38%,与正常乳腺组织相比差异有显著性意义(P<0.001)。②CyclinD2和CDK4的表达与乳腺癌的组织学分级,核分裂数有关(P<0.001),和局部复发有关(P<0.05),而与患者年龄,肿瘤大小、组织类型无关(P>0.05),CDK4的表达与乳腺癌的淋巴结转移,雌激素受体状态也有密切关系(P<0.01)。③P27kip1的表达缺失,与肿瘤大小、组织学分级,核分裂数,有无淋巴结转移以及局部复发均有关系(P<0.05),而且P27kip1的表达与CyclinD2和CDK4的表达呈显著负相关(P<0.01)。结论:CyclinD2和CDK4的表达与乳腺癌的发生、发展有关,而P27kip1的表达缺失和CDK4的异常表达与乳腺癌的侵袭、转移及复发有关,可作为判断乳腺癌生物学行为和预后的重要指标。 AIM:To investigate the expression levels of cyclinD2 and cyclin dependent kinase 4(CDK4) protein in breast carcinoma,and probe into their relationship with expression of P27kip1 protein and clinicopathological indexes as well as the prognostic significance in breast carcinoma. METHODS:The specimens of breast carcinoma were examined pathologically in the Department of Pathology,College of Medical Sciences,Hebei University of Engineering from January 1996 to December 2000.The S P immunohistochemical technique was used to detect the expressions of cyclinD2,CDK4 and P27kip1 of paraffin sections in 96 cases of breast carcinoma and 18 cases of adjacent non tumor normal breast tissues.The relationship of expression of cyclinD2,CDK4 with P27kip1 protein and with clinicopathological indexes were analyzed. RESULTS:① The positive expressions of cyclinD2,CDK4 and P27kip1 protein in 96 cases of breast carcinoma were 42% ,54% and 38% respectively,significantly different from those in normal breast tissues(P< 0.001).② The expression levels of both cyclin D2 and CDK4 protein in breast carcinoma were significantly associated with histological classification,mitotic index(P< 0.001),and local recurrence(P< 0.05),and little associated with age,tumor size and histological type(P >0.05).The expression level of CDK4 in breast carcinoma was related to lymph node metastases and estrogen receptor(ER) status(P< 0.01).③ The loss of P27kip1 protein expression in breast carcinoma was significantly associated with tumor size,histological classification,mitotic index,lymph node metastases and local recurrence(P< 0.05);however,there was a negative correlation of P27kip1 with cyclinD2 and CDK4 expression in breast carcinoma(P< 0.01). CONCLUSION:The expressions of cyclinD2 and CDK4 protein play an important role in the carcinogenesis and progression of breast carcinoma.The loss P27kip1 expression and abnormal expression of CDK4 is associated with the invasion,metastases and local recurrence,and can be served as the important indicators for biological behavior and prognosis of breast carcinoma.
