透明晶状体摘除联合人工晶状体植入治疗高度近视的疗效观察 被引量:4

摘要 目的 :探讨采用超声乳化联合人工晶状体植入术治疗高度近视的疗效。方法 :对 31例 (41只眼 )高度近视眼经透明角膜切口或巩膜隧道切口行超声乳化联合人工晶状体植入术 ,平均年龄 (44 2 1± 5 33)岁 ,术前最佳矫正视力为指数 / 30cm~ 0 6 ,平均等效球镜度数为 (- 15 5 4± 4 13)D ,平均眼轴长度为 (2 8 4 5± 5 13)mm ,植入人工晶状体屈光度 (- 7 0~+14 5 )D。结果 :所有病例术后矫正视力均超过术前最佳矫正视力 ,术后裸眼视力达到或超过术前最佳矫正视力者 31只眼(75 6 % ) ,术后裸眼视力达到或超过 0 5者 18只眼 (43 9% )。术后屈光度平均值为 (- 1 85± 1 32 )D ,37只眼 (90 2 % )在设计预矫屈光度± 1D内 ,39只眼 (95 1% )在预矫屈光度± 2D内。术后 6~ 2 4个月 (平均 15个月 )随访期中 ,5只眼 (12 2 % )发生后囊膜混浊。未发现视网膜脱离、黄斑囊样水肿、眼底出血、青光眼、人工晶状体偏位等并发症。结论 :超声乳化透明晶状体摘除联合人工晶状体植入术是治疗高度近视安全、有效的方法 ,具有术后视力好、并发症少、预测屈光状态准确性及稳定性较好等优点。其远期疗效有待观察。 objective:to investigate the clinical therapeutical values and safety of clear lens extraction with phacoemusification implantation of posterior chamber intraocular lens (pc-iol) to correct high myopia.methods:thirty-one patients (41eyes) with high myopia and clear lens had undergone phacoemulsification and posterior chamber intraorcular lens (pc-iol) implantation through clear cornea incision or scleral tunnel incision.the average age of patients was (44.21±5.33) years.the preoperative best corrected visual acuity (bcva) was fc/30cm to 0.6,the mean myopic diopter was (-15.54±4.13)d,the average axial length was (28.45±5.13)mm and the dioptric power of pc-iol was-7.0d to +14.5d.results:the bcva were improved in all cases after operation.the uncorrected visual acuity (ucva) in 31 eyes (75.6%) were better than their bcva before surgery.the ucva in 18 eyes (43.9%) achieved 0.5 or better postoperatively.the mean myopia after operation was (-1.85±1.32)d,the refraction of 37 eyes (90.2%)were within the range of the predicted diopter ±1d,the refraction of 39 eyes(95.1%) were within the range of the predicted diopter ±2d.there was no complication during operation.the postoperative complication was posterior capsular opacification (5 eyes,12.2%).retinal hole or detachment,macular edema,glaucoma and iol decentration were not observed during follow-up period (6-24 months,average 15 months).conclusions:clear lens emulsification with pc-iol implantation to correct high myopia is safe and effective as a refractive surgery.it has advantages in low incidence of complications,good bcva after operation,high predictability and stability.longer follow-up is needed to assess the complications completely.
