摘要
目的观察鼠表皮细胞膜片与猪去细胞真皮基质复合移植修复全层皮肤缺损的可行性和效果。方法以SD乳鼠的第3代表皮细胞制备细胞膜片,高渗盐水/氢氧化钠法制备猪去细胞真皮基质。36只SD大鼠全层皮肤缺损创面,分别行去细胞真皮基质与表皮细胞膜片复合移植和单纯表皮细胞膜片移植。结果两组创面术后均未见对移植物的急性期免疫排斥反应,第2、4、6周复合植皮组移植物成活率分别为(77.05±4.69)%、(83.12±5.13)%、(90.66±4.87)%,与细胞膜片移植组差异无统计学意义(P>0.05),但移植物收缩率显著减低,分别为(9.84±2.33)%、(18.97±3.40)%、(25.92±3.11)%(P<0.05)。复合植皮组上皮化良好,胶原纤维排列有序,基底膜结构完整。结论复合移植适用于修复全层皮肤缺损,能改善创面愈合质量。
Objective To observe the effect and practicability of recombinant composite skin graft consisting of acellular porcine dermal matrix (APDM) combined with cultured rat epidermal cell sheet to repair wound.Methods Using the third passage epidermal cells from newborn SD rats,epidermal cell sheet was harvested.APDM was produced by hyperosmotic saline/sodium hydroxide method.Thirtty-six SD rats with full-thickness skin defect were randomly divided into 2 groups,then composit skin (APDM+epidermal cell sheet),and epidermal cell sheet were adopted to cover the skin defect respectively.Results There was no obvious sign of acute immune rejection to the graft.The graft survival rate in the composit skin grafting group was (77.05±4.69)%,(83.12±5.13)%,(90.66±4.87)% at the 2nd,4th,6th week respectively.Compared with the epidermal cell sheet group,there was no significant difference between the two groups (P>0.05),but the contraction rate of the grafts in composite skin group was (9.84±2.33)%,(18.97±3.40)%,(25.92±3.11)% at the 2nd,4th,6th week respectively,which was significantly lower than in the epidermal cell sheet group (P<0.05).There was full differentiation of epithelium,orderly collagen arrange.Basal membrane of the skin could be identified by immunohistochemical stain.Conclusion The wound covered with composite skin graft of APDM combined with cultured epidermal cell sheet could improve wound healing quality.
