摘要
目的 为呼吸衰竭患者寻求解决CO2 潴留的合理治疗方法 ,验证小流量静脉 静脉外膜肺 (VV ECMO)排出CO2 的功能。方法 绵羊 2 0只 ,随机分为ECMO组及供血组各 10只 ,ECMO组实施VV ECMO ,VV ECMO流速 30ml/ (kg·min)运转 180min ,测定运转前、运转 15、30、6 0、90、12 0、180min动态测定膜氧合器前后的血气 ,监测股动脉血气、动脉血压、脉搏、肛温及活化凝血时间 (ACT)。结果 实验过程中平均动脉血压、脉搏、肛温均维持在正常范围 ;股动脉血气指标正常 ;膜氧合器后 pH、PO2 显著高于膜氧合器前 (P <0 0 1) ,膜氧合器后PCO2 显著低于膜氧合器前 (P <0 0 1)。结论 小流量VV ECMO运转时动脉血气维持在正常范围。VV ECMO流速在 30ml/ (kg·min)时 ,可有效摄取O2和排出CO2 。
Objective In order to test the effect of low flow rate venovenous extracorporeal membrane oxygenation (VV-ECMO) on removing CO_2 in healthy sheep.Methods Twenty health sheep were randomly divided into ECMO group and blood-supplied group.Each group had 10 sheep.Standard procedures for venovenous ECMO(30?ml/kg·min) with required mechanical ventilation were used in ECMO animals.The arterial blood pressure,pulse,temperature and ACT were monitored.The blood gas analysis from femoral artery,pre-oxygenator and post-oxygenator were measured at pre-ECMO and 15,30,60,90,120 and 180 min during VV-ECMO.Results During the VV-ECMO period,the arterial blood pressure,pulse,temperature and ACT of the animals were maintained at normal range.The blood gas analysis from femoral artery remained at normal range.Compared to pre-oxygenator,the pH and PO_2 from post-oxygenator were significantly increased,while the PCO_2 decreased.Conclusion During the low flow rate VV-ECMO,the blood gas analysis from femoral artery remained at normal range.The membrane oxygenator can oxygenate and remove CO_2 effectively by using a low flow rat VV-ECMO(30?ml/kg·min) for a period of 180 minutes.
