播散性血管内凝血患者红细胞膜磷脂的改变及其意义

摘要 目的探讨播散性血管内凝血(DIC)患者红细胞膜磷脂是否参与DIC的发病过程。方法以红细胞体外溶血后所获得的磷脂代替活化部分凝血活酶时间试验中的脑磷脂,以正常人的混合血浆为基质血浆,测定凝血时间。结果该方法简便,重复性好,DIC患者治疗前的红细胞膜磷脂APTT明显较正常人延长,经针对病因和抗凝治疗后, DIC患者的APTT可恢复至接近正常。结论DIC患者红细胞膜磷脂可能参与DIC发病过程中高凝状态的形成,以DIC 患者红细胞膜磷脂测定APTT简便易行,可作为DIC治疗过程中监测指标之一。 Objective To explore whether phospholipids in erythrocyte membrane in patients with disseminasted intravascular coagulation(DIC) is involved in the course of this disease. Methods A replacement of phospholipids deriving from the lysed erythrocytes in vitro for kephalin and base plasma deriving from a plasma mixture of normal individuals were employed in activated partial thromboplastin time( APTT) assay. The clotting time was determined. Results APTT was stable and could be repeated well in normal controls, while in patients with DIC prior to treatment, APTT was significantly prolonged, after lifting the causes and anticoagulant therapy, APTT in DIC patients could be close to normal. Conclusion Phospholipids in erythrocyte membrane in patients with DIC may be in volved in the formation of hypercoagulation state in DIC, APTT can be used as an index of treatment in DIC when phospholipids from lysed erythrocyte was employed in APTT assay.
