摘要
目的:探讨不同剂量氯氮平对精神分裂症患者认知功能的影响以及治疗前后P300电位的变化对其评估作用。方法:将87例精神分裂症患者,按氯氮平高、中、低剂量分为3组治疗,于治疗前和治疗6周后分别进行P300电位测定和阴性与阳性症状量表(PANSS)评定,并将3组结果加以比较。结果:高、中剂量氯氮平治疗后P300电位成分中N2,P3波潜伏期较治疗前缩短,高剂量组P3波幅治疗后较治疗前明显升高,差异具有显著性意义(P<0.01或P<0.05),而低剂量组治疗前后P300电位差异无统计学意义(P>0.05);与其他两组P300电位结果比较,高剂量组治疗6周后N2,P3波潜伏期缩短和P3波幅升高明显,差异存在统计学意义(P<0.01或P<0.05);3组治疗后PANSS量表的阳性、阴性及一般病理分均低于治疗前,差异具有统计学意义(P<0.01),与3组治疗后P300电位测定结果相关分析,仅见高剂量氯氮平治疗后的一般病理分和P3波潜伏期呈显著正相关(r=0.566,P=0.022)。结论:高、中剂量氯氮平能改善精神分裂症患者的认知功能,高剂量氯氮平疗效较显著。
AIM:To explore the effect of clozapine of different dosages on the cognitive function in patients with schizophrenia, and investigate the accessing role of the changes of P300 potential before and after treatment. METHODS:Eighty-seven patients with schizophrenia were divided into 3 groups according to the high,middle and low dosage of clozapine.P300 potential test and positive and negative symptom scale(PANSS) test were conducted to all the patients before and 6 weeks after treatment, and the results among the 3 groups were compared. RESULTS:The latent periods of N2 and P3 of P300 potentials after treatment in the high and middle dosage groups were much shorter than those before treatment,and the amplitude of P3 in the high dosage group was significantly increased as compared with that before treatment (P< 0.01, P< 0.05).But there was insignificant difference in the P300 potentials in the low dosage group before and after treatment(P >0.05).Compared with the results of P300 potentials after treatment in the other two groups,the latent periods of N2 and P3 were much shorter and the amplitude of P3 was higher in the high dosage groups 6 weeks after treatment(P< 0.01,P< 0.05). The scores of negative symptoms,positive symptoms and average pathology of PANSS scale in the three groups after treatment were all obviously lower than those before treatment(P< 0.01).Correlation analysis of the results of P300 potential determination after treatment in the 3 groups showed that the latency of P3 of P300 potentials in the high dosage group had significantly positive relation with the score of average pathology with PANSS scale(r=0.566,P=0.022). CONCLUSION:High and middle dosage clozapine can improve the cognitive function of patients with schizophrenia, and the efficiency of high dosage is more significant.
