^(99)Tc^m-TRODAT-1 SPECT脑显像对早期帕金森病患者纹状体及多巴胺转运蛋白功能变化的评估价值

摘要 目的:探讨Tcm-TRODAT-1SPECT脑显像在早期帕金森病(Parkinsondis-99ease,PD)评估中的价值,为实施有效的神经保护治疗提供理论依据。方法:对10例早期PD患者(Hoehn-YahrⅠ级)和10例年龄匹配正常对照者分别进行Tcm-TRODAT-1SPECT脑显像,应用计算机感兴趣技术99分别计算早期PD患者和正常对照者双侧纹状体/小脑的放射性计数比值,并对结果进行比较分析。结果:早期PD患者的患侧肢体对侧纹状体/小脑的放射性计数比值(1.53±0.09)较正常对照者双侧均值为1.74±0.05)明显降低,差异(有显著性意义(t=6.802,P=0.000),较同侧(1.67±0.07)也有降低,差异有显著性意义(t=4.035,P=0.0008),并且患侧肢体同侧纹状体/小脑的放射性计数比值较正常对照者也有降低,差异有显著性意义(t=2.896,P=0.0096)。结论:早期PD患者患侧肢体对侧纹状体DAT的功能明显降低,同侧纹状体的功能也有降低,99Tcm-TRODAT-1SPECT脑显像有助于PD的早期诊断。 AIM:To investigate the diagnostic value of 99Tcm TRODAT 1 brain single photo n emission computerized tomography(SPECT) imaging in the early stage of Parkinso n disease(PD) in order to provide theoretical evidence for neuroprotection. METHODS: 99Tcm TRODAT 1 SPECT brain imaging was performed on 10 patients wit h early stage PD(Hoehn Yahr stageⅠ) and 10 age matched healthy control subje cts.The radiation ratios of striatum to cerebellum were calculated by region of interest(ROI) technique.The results were analyzed and compared. RESULTS:The radiation ratios of striatum to cerebellum contralateral to the af fected limb of patients with early stage PD were lower than those in the health y control subjects(1.53±0.09 vs 1.74±0.05,t=6.802,P=0.000),and were lower than those of ipsilateral(1.67±0.07,t=4.035,P=0.000 8).Moreover,the radiation ratio s of striatum to cerebellum ipsilateral to the affected limb of patients with ea rly stage PD were lower than those in the healthy control subjects(t=2.896,P=0. 009 6). CONCLUSION:DAT function of striatum contralateral and ipsilateral to the affec ted limb of early PD patients is reduced. 99Tcm TRODAT 1 SPECT brain imaging i s helpful in the early diagnosis of PD.
