急性心肌梗塞内分泌代谢改变对糖代谢的影响及其临床意义 被引量:30

The influence of endocrine changes on the serum glucose metabolism in acute myocardial infarction and their clinical significance

摘要 本文研究了34例急性心肌梗塞(AMI)患者血浆儿茶酚胺(CA)、游离三碘甲状腺原氨酸(FT3)β.内啡吠(β-Ep)、生长激素(GH)、胰岛素(INS)、胰高血糖素(Glu)和血糖(G)的变化,其中14例入院2周内进行连续测定,发现AMI后48小时内血浆去甲肾上腺素(NE)、肾上腺素(E)、β-Ep、GH、Glu、G均明显高于健康对照组,FT3、INS明显低于健康对照组,并且心功能不全越重,NE、EFT3、β-Ep、INS及G变化就越大。相关性分析显示:G与NE、E、β-Ep呈明显正相关,与FT3和INS呈明显负相关。14例患者短期观察也发现其G代谢紊乱受众多因素影响,并随时间变化而变化,提示,AMI患者血糖浓度改变有助于病情严重度的估价。 Abstract The changes of plasma catecholamine (CA), free triiodothyronine(FT3),β-endorphin(β-Ep), growth hormone(GH), insulin, glucagon and serum glucose were studied in 34 patients with acute myocardial infarction(AMI),and were serially measured in 14 cases of them within 2-week hospital days. The results showed that plasma noradrenaline(NE), adrenaline(E),β-Ep,GH,glucagon and serum glucose were significantly higher and FT3 ,insulin were significantly lower than those of normal control group within 48 hours after the onset of AMI. It also showed that the more severe degree of heart dysfunction was, the greater the changes of NE,E, FT3,β-Ep,insulin and serum glucose were. These were positive correlations between the serum glucose and NE,E or β-Ep,but negative correlations between the serum glucose and FT3 or insulin, A short term observation in 14 cases also revealed that many factors could influence glucse metabolism and in the different period,the glucose level would be different. The present study suggests that the changes of serum glucose level in cases of AMI can help to evaluate the severity of AMI.
