咪达唑仑和双异丙酚静脉麻醉诱导对循环系统影响的比较

摘要 目的比较咪达唑仑和双异丙酚全麻诱导期,对支撑喉镜刺激引起的循环反应过程的影响。方法选择择期全麻下行声带息肉摘除手术的患者21例,随机分为两组,A组11例,给予咪达唑仑02~03mg·kg1;B组10例,给予双异丙酚3~4mg·kg1诱导。观察两组患者诱导前(T0)、插管即刻(T1)、插管后3min(T2)、插管后10min(T3)各时间心率(HR)、平均动脉压(MAP)、心率与收缩压乘积(RPP)等循环变化。结果A组T1、T2和T3各阶段的HR、MAP、RPP均明显低于B组各时间值(P<001)。结论咪达唑仑可以较好地抑制支撑喉镜刺激引起的循环系统反应,麻醉诱导平稳,术毕能较快清醒。 Objective To observe the effects of midazolam and of disoprofol on circulation at different anesthesia phases during framing laryngeal endoscope. Methods Twenty-one patients needed systemic anaesthesia for polyp resection on vocal cord were randmised into two groups. Group I received midazolam 0.2-0.3 mg·kg^-1 ; Group II received disoprofol 3-4 mg·kg^-1 . Heart rate(HR), mean artery pressure(MAP), and rate pressure product(RPP) at before the anesthesia induction(T_0 ), the time during intubation(T_1), 3 min after the intubation(T_2), 10 min after the intubation(T_3) phases were monitored. Results No difference of HR, MAP, RPP were observed within the two groups before intubation. The values of HR, MAP, RPP at T_1, T_2 and T_3 phases in Group Ⅰ were significantly lower than in Group Ⅱ(P<0.01). Conclusion Midazolam suppress the circulation irritation by tracheal intubation better. It has the advantages of stable anesthesia induction and of less time for resuscitation.
