摘要 目的 探讨HSP7 0基因转染对大鼠脓毒症早期外周血细胞因子的影响。方法 采用盲肠结扎穿孔 (cecalligationpuncture,CLP)制作脓毒症的病理模型并转染HSP7 0基因,分析HSP7 0基因转染前后外周血细胞因子TNF-α,IL-1β,IL-6,IL 1 0的变化。结果 CLP组血清TNF-α浓度 3h开始升高, 6h达到峰值 (tTNF α= 1 6. 5 0 6, P< 0. 0 1 );IL 1β浓度 6h显著增高, 2 4h达到峰值 (tIL 1β=2 2. 5 3 6, P< 0. 0 1 ); IL 6浓度 3h显著升高, 1 2h达到峰值 (tIL 6 = 3 3. 9 7 7, P< 0. 0 1 );IL 10浓度2 4 h显著升高, 4 8 h达到峰值 (tIL 10 = 1 5. 0 5 6, P< 0. 0 1 ), 7 2 h恢复到对照组水平。HSP7 0基因转染组 6h的TNF α, 1 2h的IL 6, 2 4h的IL 1β血清浓度与对照组相比都没有明显的差异 (tTNF α=0. 580; tIL 6 = 1. 5 8 9; tIL 1β= 0. 8 0 2; P> 0. 0 5 )。 4 8h的IL 1 0的血清浓度与对照组相比差异有显著性 (tIL 10 = 3. 7 9 6, 0. 0 1 <P< 0. 0 5 )。结论 HSP 7 0基因转染能下调外周血促炎细胞因子表达,不影响抗炎细胞因子的表达。提示HSP 7 0基因转染具有抗炎作用,对于防治脓毒症及多脏器功能不全具有潜在的临床应用价值。 Objective To investigate the effect of HSP70 gene transfection to peripheral blood cytokines in the early phase of sepsis in rats. Methods A rat model of sepsis was established by cecal ligation and puncture(CLP),and adenovirus-mediated HSP70 gene transfection was performed in the CLP rats. Serum TNF-α,IL-1β,IL-6,IL-10 were measured before and after HSP70 gene transfection. Results In CLP rats, serum TNF-α increased at 3h and peaked at 6h(t_(TNF-α)=16.506, P<0.01). Serum IL-1β began to elevate at 6h and reached the peak at 24h(t_(IL-1β)=22.536, P<0.01). Serum IL-6 was significantly higher than control group at 3h, and peaked at 12h(t_(IL-6)=33.977, P<0.01). IL-10 began to increase at 24h, reached its peak at 48h(t_(IL-10)=15.056;P<0.01), and returned to the level of control group at 72h. In HSP70 gene transfection rats, the pro-inflammation cytokine level of TNF-α at 6h,IL-6 at 12h,IL-1β at 24h was (inhibited),but anti-inflammation cytokine level of IL-10 at 48h was not inhibited. (Conclusions) HSP70 gene transfection might down-regulate the overexpression of pro-inflammatory cytokines in the body, and the expression of anti-inflammation cytokines was not inhibited. These results suggested thattranfection of HSP70 gene can provide protective effects against sepsis. HSP70 gene transfection might be of clinical relevance in the (prevention) and treatment of sepsis and MODS.
