摘要
目的 探讨实时三维彩色多普勒技术评价偏心性二尖瓣反流量的价值。方法 偏心性二尖瓣反流患者 39例,分别使用二维彩色多普勒、频谱多普勒及实时三维超声计算反流束面积 (JA)、反流束面积与左房面积比值(JA/LAA)、反流量(RV)、反流分数(RF)及三维反流束的彩色多普勒信号容积 (JV),并将结果进行分析比较。结果 ①JA与RV和RF呈中度相关(r=0. 49和r=0. 31), JA/LAA与RV和RF呈中度相关 (r=0. 57和r=0. 51)。②JV与RV呈高度相关(r=0. 88),与RF呈中度相关(r=0. 67)。③三维法和频谱法评估反流程度一致性好(Kappa值 76. 52% )。结论 实时三维彩色多普勒超声为定量评价偏心性二尖瓣反流提供了一种简便、准确、可靠的方法。
Objective To explore the value of assessing the eccentric mitral regurgitant volume by real time three-dimensional color Doppler in clinical applications.Methods Thirty-nine patients with simple eccentric mitral regurgitation were studied.Regurgitant jet area(JA),the ratio of regurgitant jet area and left atrial area(JA/LAA),regurgitant volume(RV),regurgitant fraction(RF),regurgitation jet volume(JV) were measured by means of two dimensional color Doppler,spectral Doppler and real time three-dimensional color Doppler respectively.All the results were further analyzed and compared.Results ①JA correlated moderately with RV and RF (r=(0.49),(0.31),respectively),and JA/LAA correlated moderately with RV and RF (r=(0.57),(0.51),respectively).②JV correlated strongly with RV(r=(0.88)),and moderately with RF (r=(0.67)).③Results from real time three-dimensional color Doppler were in agreement with those from spectral Doppler for grading the severity of eccentric mitral regurgitation (Kappa value=(76.52%)).Conclusions Real time three-dimensional color Doppler is a convenient,accurate and reliable measure in quantifying eccentric mitral regurgitation.
