摘要
目的初步总结放射性粒子定向植入治疗计划系统在治疗口腔颌面部恶性肿瘤中的应用价值。方法对5 0例口腔颌面部恶性肿瘤患者,实施了放射性粒子定向植入治疗(分别为术中即刻种植、术后经皮穿刺种植、单纯粒子种植)。治疗实施前设计种植计划。计划设计要点为①采集靶区图象所有图像资料来自术中所见的肿瘤范围或病变区CT图像。②图象处理及制定治疗计划将所采集图像输入计算机,应用治疗计划系统软件,在靶区模拟粒子种植,绘制出等剂量曲线并检测剂量分布,调整粒子位置使之分布合理,最后确定治疗计划。结果治疗计划完成后,以TPS的检测软件进行检测,所用处方剂量涵盖的范围均包括了95 %以上的靶面积或靶体积,达到了靶剂量区与病损区高度适形的要求。结论治疗计划系统为放射性离子定向植入治疗口腔颌面部肿瘤提供了重要的参考依据并为临床资料的总结分析建立了较为完善的基础。
Objective To describe the methods developed and implemented in treating oral and maxillofacial malignant tumors with radioactive seed orient implantation according to the characteristics of the tumor and to evaluate the effectiveness and feasibility of TPS in the treatment.Methods Fifty pateints with oral and maxillofacial malignant tumors were selected and treated with radioactive seed implanting (intraoperative, postoperative or single seed implantation). The treatment plans were designed before the implantation. The design included:①image collections of target volumes. All of the images were from tumors' scope observed during operation or from CT.②Image management and design completion. All of the images were transfered into a computer. By using the TPS software, seed implantation was simulated on the target volume, and then the equivalent dose curve was generated. After adjusting the seed position, the treatment planning was completed.Results After completion of the treatment plan, we detected the plan using TPS software and found that the prescription dose could cover ~95% of the target proportion or target volume. Target dose areas are well suited to the lesions.Conclusion TPS provides important reference for radioactive seed orient implantation in the treatment of oral and maxillofacial malignant tumor.
