摘要
目的:探讨狼疮肾炎患者对糖皮质激素(以下简称激素)治疗反应不同的分子机制。方法:采用逆转录-聚合酶链反应(RT-PCR)方法检测正常对照组(10例)和狼疮肾炎组(28例,其中激素敏感组10例、激素依赖组10例、激素抵抗组8例)外周血单个核细胞(PBMCs)内GRα与GRβmRNA表达水平的变化。结果:与正常对照组相比,狼疮肾炎患者三组中GRαmRNA的表达水平均降低,差异有显著性(P<0.05);GRβmRNA在激素抵抗组中表达明显增高,差异有显著性(P<0.01),而在激素敏感组、激素依赖组中表达无明显差异(P>0.05)。结论:狼疮肾炎患者糖皮质激素受体GRα、GRβmRNA表达水平的变化,导致GRα与GRβ比例失调,使组织细胞对激素的敏感性降低,因此,导致狼疮肾炎的患者对糖皮质激素治疗反应不同,狼疮肾炎患者出现激素抵抗可能与GRβmRNA表达水平增高有关。
Objective: To investigate the mechanism of steroid resistance in patients with lupus nephritis. Methods: The expression of glucocorticoid receptor-alpha and beta (GRα and GRβ) mRNA in peripheral blood mononuclear cells (PBMC) from ten normal controls and ten steroid-sensitive, ten steroid-dependent, eight steroid-resistant patients with lupus nephritis were detected by reverse transcription polymerase chain reaction (RT-PCR). Results: Compared to normal, the expression of GRα mRNA in the three groups of lupus nephritis were significantly decreased ( P <0.05). The expression of GRβ mRNA was highly increased in steroid-resistant lupus nephritis ( P <0.01), whereas no significant difference was detected in steroid-sensitive and steroid-dependent lupus nephritis. Conclusion: The changed expression of glucocorticoid receptors-alpha and beta mRNA in lupus nephritis leads to decreased ratio of GRα to GRβ and reduced sensitivity to glucocorticoid in cells. Therefore, the phenomenon of steroid-resistance in some patients with lupus nephritis might be related to the increased expression of GRβ mRNA in PBMCs.
