血清BAP和PSA测定在前列腺癌骨转移诊断中的价值 被引量:9

摘要 我们分析、比较了骨源性碱性磷酸酶(BAP)和前列腺特异抗原(PSA)对前列腺癌骨转移的诊断意义,并探讨了二者在前列腺癌骨转移分级中的应用价值。对68例前列腺癌病人进行骨显像,根据骨显像结果及临床表现分为骨转移组与非骨转移组。骨显像依Soloway分级标准分为五级(M0—M4)。对所有病人进行血清BAP与PSA检测,各组之间BAP值及PSA值比较采用t检验。对转移级别的升高与BAP值之间进行相关分析。结果显示骨转移的级别升高与BAP呈正相关。(r=1.1083,P<0.01)。PSA仅在M0与M1—M3之间有显著差异。BAP对前列腺癌骨转移最佳检测范围为25—35u/L,阳性预测率为96.6%,阴性预测率为75%。BAP与PSA值分别为35u/L和20ng/mL时,骨转移阳性预测率为97.5%。BAP与PSA值分别为25u/L和20ng/mL时,阴性预测率达73.9%。由此我们得出:1.BAP预测前列腺癌骨转移比PSA有效,并且BAP对前列腺癌骨转移的分级有诊断价值;2.BAP与PSA联合应用可提高前列腺癌病人骨转移的阳性预测率。 The diagnosis value of BAP and PSA in prostate cancer with bone metastases was studied and its application in the classification of bone metastases was investigated. 68 patients of prostate cancer undertook bone scintigraphy, and according to the result they were divided into 2 groups, with and without bone metastases. Furthermore the bone scan results were divided into 4 grades according to Soloway standard. BAP and PSA tests were done in all patients and the correlation was calculated among various groups. Relation was studied between BAP and bone metastases grade. The result showed a positive correlation between BAP and bone metastases grade (r = 1.1083, P less than or equal 0.01), while PSA only showed significant difference between M0 and MI-M3. The best value of BAP for prostate cancer metastases is 25-5 u/L, because the positive predictive value was 96.6% and negative predictive value was 75%. When using BAP in 35 u/L and PSA in 20 ng/mL, the positive predictive value was 97.5%. When using BAP in 25 u/L and PSA in 20 ng/mL, the negative predictive value was 73.9%. We can conclude that BAP is more effective in predicting bone metastases in prostate cancer than PSA, and has more diagnosis value in the grading of metastases. And the combination of BAP and PSA can increase the positive predictive value of bone metastases of prostate cancer.
