摘要
目的:探讨甲状腺肿块与颈前肌粘连与手术及病理的相关性,为甲状腺癌的诊断提供更为可靠的依据。方法:回顾性分析经我院超声检查并经手术及病理证实的甲状腺肿块87例,观察甲状腺被膜与颈前肌回声,并与手术及病理对照。结果:甲状腺肿块与颈前肌粘连较轻者超声仅表现为结节局部甲状腺被膜亮线连续性中断,若结节侵入肌层内并可见该处肌组织回声模糊。47例甲状腺癌中,术中发现有28例(59.6%)肿块与颈前肌粘连,超声检出24例,其中符合22例,符合率为78.6%(2228);10例甲状腺炎病例,术中及超声均发现3例结节与颈前肌粘连;20例结节性甲状腺肿患者,2例以前有甲状腺手术史的患者术中及超声均发现有粘连,其余18例患者均未发现粘连;10例甲状腺腺瘤患者手术及超声均未发现粘连。本组病例中,超声对甲状腺结节与颈前肌粘连的显示符合率为81.8%(2733)。结论:高频超声能较准确显示甲状腺结节是否与颈前肌粘连,是甲状腺癌诊断的一个较为有用的指标。
Objective: To analyze the high frequency sonographic features of thyroid mass adhesion to the anterior cervical muscles and correlated with surgical and pathologic results. Materials and Methods: Eighty-seven cases of thyroid mass were examined by ultrasound and confirmed by operation and pathology. The echo of the thyroid capsule and the anterior muscle were observed and correlated with the result of operation and pathology. Result: The bright line of the thyroid capsule adjacent to the mass was discontinuous in slight adhesion between thyroid mass and anterior cervical muscles in ultrasonography. The muscle echo will be indistinct with the mass invasion. Among the 47 cases of thyroid cancer, 28 cases(59.6%) were found to have adhesion between thyroid mass and anterior cervical muscles in operation. Twenty-four cases were found to have adhesion in ultrasound examination in which 22 cases were consistent with surgery(22-28, consistent rate 78.6%). Among the 10 cases of thyroiditis, 3 cases were found to have adhesion between thyroid mass and anterior cervical muscles in operation as well as in ultrasound examination. Among the 20 cases of nodular goiter, 2 cases that had thyroid operation history were found adhesion in operation and in ultrasound examination as well, the others had no adhesion in operation and ultrasound examination. In 10 cases of thyroma no adhesion were found in operation and ultrasound examination. In our series 81.8%(27-33) of cases showed adhesion between thyroid mass and anterior cervical muscles by ultrasound. Conclusion: High frequency ultrasound can show adhesion between thyroid mass and anterior cervical muscles accurately, it is an effective method in diagnosing thyroid cancer.
