摘要
目的探讨实时三维超声心动图(RT-3DE)技术评估房间隔缺损面积及其在心动周期中的变化.方法应用RT-3DE技术对 31例房间隔缺损患者进行三维成像,结合三维数据工作站,于心动周期的P波顶点、R波顶点、T波起点、T波终点、T-P段分别测量房间隔缺损的面积,并与手术测值进行相关性分析. 结果①RT-3DE技术可对房间隔缺损面积进行测量,房间隔缺损面积在心动周期中呈动态改变,在收缩末期面积最大,舒张末期面积最小.②RT-3DE测值与手术测值相关良好,且在P波顶点测量的房间隔缺损面积与手术测值相关性最佳(r=0.92),余心动周期时相RT-3DE技术与手术测值均相关性良好(r=0.81~0.86).结论实时三维超声技术为房间隔缺损面积的测量提供了准确可靠的新方法.
Objective To investigate the accuracy in quantitating the atrial septal defect area and the dynamic change by real time three dimensional echocardiography(RT 3DE). Methods Three dimensional imaging was acquired in the patients with atrial septal defect by RT 3DE, combinated with three dimensional workstation to dissect the en face of atrial septal defect for measuring its area in the peak of P wave, the peak of R wave, the initial point of and the destination of T wave, the period of P T respectively. Parameters derived by RT 3DE were compared with surgery results. Results ①The atrial septal defect area were changed during cardiac cycle, with the maximal area at end systole and the minimal area at end diastole. ②There was excellent correlation between RT 3DE and surgery for the area of atrial septal defect at P wave(r= 0.92 ).During the rest cardiac cycle, the area showed also good correlations between RT 3DE and surgery(r= 0.81 - 0.86 ). Conclusions RT 3DE provided an accurate and feasible method in measuring the atrial septal eefect area.
