血管回声跟踪技术定量评价正常人颈动脉弹性 被引量:135

Quantitative evaluation of carotid elasticity in normal adults by echo tracking technique

摘要 目的探讨血管回声跟踪技术定量评价健康志愿者颈动脉弹性参数及其影响因素,确定相关参数的正常值范围.方法对143位健康志愿者[男性72例,女性71例,平均年龄(47.2±14.9)岁]应用血管回声跟踪技术获取双侧颈动脉的弹性系数(Ep)、僵硬度(β)及顺应性(AC)三项指标,并分析其与年龄、性别、心率和血压的关系.结果①年龄与Ep和β显著正相关而与AC显著负相关,收缩压与Ep显著正相关,性别、心率和舒张压与这些参数相关性不显著.②在20~29岁、30~39岁和40~49岁组中,随年龄增加Ep和β增高而AC减低;但在50~59岁和≥60岁组,各参数与40~49岁组之间差异无显著性意义,提示年龄对<40岁者颈动脉弹性参数影响显著,而对≥40岁者影响不显著.③ Ep、β和AC的正常值在≥40岁者分别为 117.97±31.04、8.67±2.23 和 0.74±0.21,而在<40岁者随年龄不同而异.结论应用血管回声跟踪技术能够准确获取颈动脉性能参数,为临床定量评价颈动脉弹性提供了一种新的手段. Objective With echo tracking technique, the normal values of carotid stiffness and compliance and the related influencing facts were determined in normal adults for their potential clinical applications. Methods The study population included 143 normal adults with 72 males and 71 females at a mean age of ( 47.2 ± 14.9 )years. The changes of the carotid diameter during cardiac cycles were obtained and the quantitative parameters of pressure strain elastic modulus(Ep), stiffness(β) and the arterial compliance(AC) were calculated automatically. Their possible influencing clinical variables such as age, gender, heart rate and blood pressure were analyzed. Results ① Age showed the strongest correlation with all the parameters while systolic blood pressure showed a significant correlation with Ep. Gender, heart rate and diastolic blood pressure showed no correlation with these parameters. ② The subjects were divided into 5 age groups and the parameters among the age groups were compared. In subjects <40 years old, Ep and β increased while AC decreased with the increase of age. In subjects ≥40 years old, Ep, β and AC showed no significant change with age. ③ The normal values of Ep, β and AC in subjects ≥40 years old were 117.97 ± 31.04 , 8.67 ± 2.23 and 0.74 ± 0.21 respectively, while these parameters changed with age in subjects <40 years old. Conclusions The stiffness and compliance of carotid were correlated significantly with age, and the echo tracking technique may be a promising modality for noninvasive estimation of arterial elastic function.
