宫颈鳞状细胞癌中E-钙黏附素的表达研究

摘要 目的:研究E -钙黏附素(E -cadherin ,E -Cad)在宫颈鳞状细胞癌中的表达及其与浸润和转移的相关性。方法:应用免疫组化方法检测4 8例宫颈浸润性鳞状细胞癌患者手术切除石蜡包埋标本中E -Cad的表达,以正常、炎症、非典型性增生的宫颈组织各30例作为对照。结果:①从正常宫颈组织发展到浸润性鳞状细胞癌,E -Cad阳性表达率呈下降趋势,P <0 .0 1。②E -Cad在无淋巴结转移的原发癌灶中阳性表达率是6 1. 76 % ,明显高于有淋巴结转移的癌灶(14 . 2 9% ) ,P <0 . 0 1。③在原发癌灶和淋巴结转移癌灶中、E -Cad阳性率分别是14 .2 9%和5 8. 82 % ,P <0 .0 5。结论:E -Cad的异常表达均促进了其恶性细胞的浸润性生长。E -Cad的低表达可促进恶性肿瘤细胞的转移,E -Cad在转移灶中的表达提示其与恶性肿瘤在转移灶中的生长有着密切关系。 Objective: To detect the expression of E-cadherin in cervical squamous cell carcinoma and its correlation with infiltration and metastasis.Methods:Experession of E-cadherin in 48 cervical infiltrating squamous cell carcinoma was detected by immunohistochemical methods.Controls were recruited from normal (n=30),inflammatory (n=30) and atypical hyperplastic cervix (n=30).Results:①Compared with normal cervix,positive incidence of E-cadherin decreased in cervical infiltrating squamous cell carcinoma (P<0.01).②Positive incidence of E-cadherin in carcinomas without lymphoid metastasis (61.76%) was markedly higher than that with lymphoid metastasis (14.25%) (P<0.01).③Positive incidence of E-cadherin in primary and lymphoid metastatic carcinoma was 14.29% and 58.82%,respectively.(P<0.05).Conclusion:Abnornal expression of E-cadherin accelerates infiltrative growth of malignant cells.Lower expression of E-cadherin promotes metastasis of malignant cells.Different expression of E-cadherin in the metastasis indicates its association with growth of malignant carcinoma in the metastasis.
